Graft Rebeating

Graft rebeating is the return of spontaneous cardiac activity in a transplanted donor heart after implantation and restoration of blood flow. During transplantation, surgeons release vascular clamps to reperfuse the graft, allowing oxygen and substrates to reach ischemic myocardium; as cellular metabolism and electrical conduction recover, the heart may resume coordinated contractions, sometimes with pacing or pharmacologic support. Rebeating provides an immediate functional indication that the graft is recovering and helps guide the transition from cardiopulmonary bypass to independent circulation. Its timing and strength are monitored in transplant surgery to assess myocardial preservation, identify dysfunction, and support decisions about postoperative management.
