Chronic pancreatitis is a progressive and irreversible inflammation of the pancreas.
While alcohol is a leading cause, chronic pancreatitis can also be idiopathic or linked to genetic factors such as PRSS1 and SPINK1 mutations. Smoking also increases the risk.
Repeated injury leads to chronic inflammation, with normal tissue replaced by fibrosis. Both acinar and islet cells are gradually lost, and destruction of islet cells impairs endocrine function, leading to diabetes.
Fibrosis causes structural changes in the gland, forming strictures. These strictures narrow the pancreatic ducts, which may fill with thick secretions or stones, further obstructing the ducts.
Blocked ducts during flare-ups can lead to pseudocysts, common in acute pancreatitis. In chronic cases, they develop less frequently and usually involve walled-off collections from acute episodes.
Loss of exocrine tissue lowers digestive enzyme and bicarbonate release in the duodenum.
As a result, nutrient absorption is impaired, leading to malabsorption and steatorrhea.
