Continuous Arteriovenous Hemofiltration

Continuous arteriovenous hemofiltration is a continuous renal replacement therapy that removes excess fluid and dissolved waste from blood, particularly when critically ill patients cannot tolerate intermittent dialysis. Blood moves from an artery to a venous return through a hemofilter, where an arteriovenous pressure gradient drives plasma water across a semipermeable membrane by convection; replacement fluid restores volume and helps regulate electrolytes. In clinical care, this technique supports patients with acute kidney injury, fluid overload, or unstable blood pressure by providing gradual solute and fluid control. Its principles also inform modern continuous extracorporeal therapies used in intensive care.

Surgery is the gold standard for accessible arteriovenious malformations (AVMs), and pre-operative embolization can simplify this procedure. We describe our approach for staged endovascular embolization and open resection of AVMs, and provide a representative clinical example highlighting the advantages of a comprehensively trained neurovascular surgeon leading a multi-disciplinary clinical team.

After puncturing the aorta through the inferior vena cava (IVC) to create an aorto-caval fistula in the mouse, solution containing a drug is infused into the IVC via the same needle, followed by incubation.
