Coughing Techniques

Coughing techniques are structured methods used to generate or modify a cough so patients can clear airway secretions while limiting fatigue, pain, or excessive airway irritation. A deep inhalation followed by brief glottic closure and forceful contraction of the respiratory and abdominal muscles creates high expiratory airflow that moves mucus toward the upper airway; huff coughing uses a more open glottis to mobilize secretions with less pressure. In clinical care, these approaches support airway clearance in patients with retained secretions, postoperative breathing limitations, or respiratory disease, and may improve ventilation, reduce secretion retention, and help patients participate safely in pulmonary rehabilitation.

The topical anesthetic lidocaine was atomized using a high oxygen flow through the working channel of a flexible intubating endoscope to achieve topical airway anesthesia for awake endotracheal intubation. We prefer this modified spray-as-you-go technique for endoscopic intubation to classical bolus application because of higher patient satisfaction and better compliance.
