Endovascular Infection

Endovascular infection is the invasion of blood vessels, heart valves, or intravascular devices by microorganisms, making it an important cause of persistent bloodstream infection and vascular injury. It develops when circulating bacteria or fungi adhere to damaged endothelium, thrombi, prosthetic material, or catheters, where biofilm formation can protect them from immune defenses and antimicrobial treatment. Resulting inflammation may produce infective endocarditis, infected vascular grafts, septic thrombosis, or embolic complications. Understanding these mechanisms supports blood-culture interpretation, imaging-based diagnosis, antimicrobial selection, and decisions about device removal or surgery, while guiding strategies to prevent healthcare-associated infections.

Histological and biochemical modifications after aneurysm endograft exclusion are still unclear. We describe a new model of endograft implantation on a murine aneurysm. Through thrombus analysis with and without persistant circulating blood stream interface, and new endovascular biomaterials evaluation, this model will help to better understand endovascular aneurysm exclusion pathobiology.

Simulator Training for Endovascular Neurosurgery

Simulation of complex, high-risk procedures is critical to the education of medical trainees. A protocol for simulator-based endovascular neurosurgery training in a controlled academic environment is described. The protocol includes stepwise guidelines for trainees of varying levels, with a discussion of the advantages and limitations of this model.

Surgery is the gold standard for accessible arteriovenious malformations (AVMs), and pre-operative embolization can simplify this procedure. We describe our approach for staged endovascular embolization and open resection of AVMs, and provide a representative clinical example highlighting the advantages of a comprehensively trained neurovascular surgeon leading a multi-disciplinary clinical team.

Catheter-based Endovascular Angioplasty for Fibrosing Mediastinitis-associated Pulmonary Vein Stenosis

2025

Pulmonary vein stenosis (PVS) is a life-threatening complication of fibrosing mediastinitis (FM), with a poor prognosis if not properly treated. Currently, catheter-based endovascular angioplasty is a promising therapeutic strategy for FM-associated PVS. This protocol primarily outlines a stepwise approach to the transcatheter procedure for FM-associated PVS.

Endovascular Treatment of Acute Ischemic Stroke Using the Stent Retriever-Assisted Vacuum-Locked Extraction Technique

Source: Schregel, K., et al. Optimized Management of Endovascular Treatment for Acute Ischemic Stroke. J. Vis. Exp. (2018).This video demonstrates the endovascular treatment of acute ischemic stroke using the stent retriever-assisted vacuum-locked extraction technique. A guiding sheath is introduced through the femoral artery to reach the clot. A catheter with a stent retriever secures and extracts the clot. Vascular imaging is done to confirm successful reperfusion.
