Hepatic Artery Perfusion

Hepatic artery perfusion is the delivery of oxygenated blood or preservation solution through the hepatic artery to support liver tissue, assess organ function, or administer treatment. In clinical and laboratory settings, controlled flow and pressure recreate arterial circulation, supplying hepatocytes and biliary structures while enabling monitoring of vascular resistance, oxygen use, and biochemical function. This approach is used in liver transplantation research, ex vivo organ preservation, and regional therapies for liver disease, where it can help evaluate graft viability, extend preservation options, and improve understanding of hepatic physiology and treatment response.

In this manuscript, we describe percutaneous isolated hepatic perfusion with simultaneous chemofiltration as treatment for unresectable liver metastases. This procedure is performed under general anaesthesia in the angiosuite by an experienced team, consisting of an interventional radiologist, a clinical perfusionist and anaesthesiologist.

The video demonstrates locoregional delivery of oncolytic viruses through hepatic arterial injection as a therapeutic approach for hepatocellular carcinoma. These viruses selectively replicate in tumor cells, inducing cell lysis and releasing tumor cell-associated antigens. This process triggers immunogenic cell death, inducing an anti-tumor response.

Auxiliary liver transplantation provides a temporary support in acute hepatic failure, until regeneration of the failing liver. The heterotopic auxiliary liver transplantation (HALT) with portal vein arterialization (PVA) renders sufficient liver function. We developed an analogous technique in the rat, to examine the influence of the portal vein arterialization on the morphology and function of the graft.
