Normothermic Ex Vivo Liver Perfusion

Normothermic ex vivo liver perfusion (NEVLP) is a machine-perfusion technique that maintains an isolated donor liver at near-physiological temperature outside the body, allowing the organ to remain metabolically active before transplantation. An oxygenated perfusate circulates through the hepatic artery and portal vein, supplying nutrients and oxygen while carrying away metabolic waste under controlled pressure and flow. During perfusion, clinicians can monitor biochemical and functional indicators, such as bile production and lactate clearance, to assess graft viability. This approach can extend preservation beyond static cold storage, support evaluation of higher-risk organs, and potentially improve access to suitable livers for transplantation.

The porcine model of liver normothermic machine perfusion (NMP), described here, can be successfully used to study NMP as a preservation strategy, a tool for viability assessment, and a platform for organ repair. It holds a high translational value, however it is technically challenging and labor-intensive.

Here we present a protocol describing oxygenated ex situ machine perfusion of donor liver grafts. This article contains a step by step protocol to procure and prepare the liver graft for machine perfusion, prepare the perfusion fluid, prime the perfusion machine and perform oxygenated normothermic machine perfusion of the liver graft.

There is a significant liver donor shortage, and criteria for liver donors have been expanded. Normothermic ex vivo liver perfusion (NEVLP) has been developed to evaluate and modify organ function. This study demonstrates a rat model of NEVLP and tests the ability of pegylated-catalase, to mitigate liver preservation injury.

A normothermic ex vivo liver perfusion (NEVLP) system was created for mouse livers. This system requires experience in microsurgery but allows for reproducible perfusion results. The ability to utilize mouse livers facilitates the investigation of molecular pathways to identify novel perfusate additives and enables the execution of experiments focused on organ repair.
