Pulmonary Hilum Clamping

Pulmonary hilum clamping is a surgical technique that temporarily occludes the structures connecting the lung to the mediastinum, including the pulmonary artery, pulmonary veins, and main bronchus. By compressing or controlling the hilum en bloc, surgeons can stop pulmonary blood flow and airflow, limiting hemorrhage and preventing contamination of the operative field. The maneuver is used in thoracic trauma, complex lung resection, and selected procedures requiring rapid control of the lung. Understanding hilar anatomy, vascular relationships, and the physiological effects of clamping helps clinicians perform the technique safely and manage complications such as hemodynamic instability or impaired oxygenation.

Hypoxic pulmonary vasoconstriction (HPV) is an important physiological phenomenon by which at alveolar hypoxia lung perfusion is matched to ventilation. The major vascular segment contributing to HPV is the intra-acinar artery. Here, we describe our protocol for the analysis of HPV of murine pulmonary vessels with diameters of 20-100 μm.

Marginating-pulmonary leukocytes exhibit unique characteristics and distinct immunological functions compared to other leukocyte populations. Here we describe selective harvesting of this subpopulation of pulmonary leukocytes, by forced perfusion of the lungs in rats and mice. Marginating-pulmonary leukocytes seem critical in determining susceptibility to blood-borne and lung-related diseases.
