Cortical Spreading Depression

Cortical spreading depression (CSD) is a slowly propagating wave of near-complete neuronal and glial depolarization followed by temporary suppression of cortical activity, making it an important model of brain excitability and injury. It begins when extracellular potassium and glutamate rise, driving neighboring cells to depolarize; ionic gradients then recover as the wave travels through gray matter at several millimeters per minute. CSD is strongly associated with migraine aura and can occur after ischemic stroke, traumatic brain injury, or intracerebral hemorrhage, where repeated waves may worsen metabolic stress. Monitoring and experimentally inducing CSD helps researchers study migraine mechanisms, secondary brain injury, and potential therapeutic targets.
