Clinical data extraction is the process of identifying and converting relevant information from medical records into structured, usable data for patient care, research, and health-system analysis. It draws on methods such as natural language processing, information retrieval, and rule-based or machine-learning models to detect entities including diagnoses, medications, laboratory results, procedures, and temporal relationships, then map them to standardized fields or vocabularies. Human review and validation help assess accuracy, completeness, and clinical context, while privacy safeguards protect sensitive information. In medicine, reliable extraction supports cohort selection, outcomes research, clinical decision support, quality measurement, and evidence generation from electronic health records.
