Enhancing Electrode Location Assessment in Cochlear Implantation via Computed Tomography Image Fusion

This protocol demonstrates a method for fusing pre-operative and post-operative computed tomography images for cochlear implant recipients. The approach may enhance the measurement accuracy of insertion depth and the center frequency of electrode array contacts. Additionally, this method has potential applications in anatomy-based fitting, an emerging area within the cochlear implant field.
We investigated whether fusing preoperative and postoperative CT images could facilitate the assessment of angular insertion depth and center frequency of electrode contacts in cochlear implant recipients. Additionally, we compared the efficacy of automatic image fusion with traditional manual methods in measuring various cochlear parameters. The most recent development include image-guided surgeries and robotic insertion tools.
These pre-and intraoperative advancement have led to more personalized and consistent clinical outcomes. The current practice of analyzing both operative CT images often involve a measurement. This require extensive training and more time, and it's prone to variability and errors, thus limiting their applicability.
And there is no sufficient literature on the role of automated views, images, in measuring the angular insertion depth of electrode contacts. The protocol incorporates an automated method, saving the time required for analysis, while minimizing the impact of electrode artifacts and postoperative CT scan imaging while measuring the angular insertion depth. Future research should focus on validating the current finding using larger cohort.
Additionally, this study introduced an innovative tool for the anatomy-based fitting field offering a valuable opportunity to investigate its effectiveness in relation to audiological patient outcomes. Begin by uploading the pre-and postoperative CT images and measuring cochlear parameters. To do so, press the import button in the software's data management module to upload both the postoperative and preoperative CT images and select the DICOM files.
Using the 3D ear cochlear module, manually define the center of the modiolus round window and cochlear metrics, including the A, B, and H values. The software then calculates the cochlear duct length using the A and B values. Open the implant assessment module.
Perform electrode contact detection, and manually adjust the detected contact points as necessary before confirming their final positions. Now, to compare the shown conventional method with a novel image fusion approach, perform image fusion. For image fusion, press the add icon in the user interface to import the preoperative CT images into the image fusion panel.
Allow the software to automatically start the registration process, aligning the preoperative CT images with the previously uploaded postoperative CT images using the automatic fusion feature, which utilizes a mutual information algorithm. Remeasure the cochlear parameters on the newly-imported preoperative image within the 3D cochlear module. Select the automatic option in the module menu to execute the automatic measurement of cochlear parameters.
