Abstract

Introduction
系统性硬化症(SSc)是一种复杂的异质、自身免疫性疾病。它可以表现为血管病,雷诺现象和皮肤和内脏纤维化1。SSc分为以下亚型:有限皮肤、漫性皮质、正子硬皮病(无皮肤参与)和SSc重叠综合征1。
SSc不是孟德利时尚遗传的,但遗传因素似乎会影响对这种疾病的易感性。不同种族的发病率不同,在有该疾病家族史的个人中,发病率增加22,3。3环境风险因素也似乎也存在,高接触二氧化硅或有机溶剂似乎增加了SSc4的发生。全球SC的患病率约为10,0001。女性比男性多受SSc影响,报告的女性:男性比例在3:1和8:1之间,发病率最高的年龄组为45-54岁。
肺是SSc6患者中第二大最常见的内脏器官。SSc有两种主要的肺表现:间质性肺病(ILD)和肺高血压7。ILD 通常是纤维化的;它发生在大约80%的SSc患者,在扩散性皮质硬皮病比在疾病的有限形式11,88更常见。肺高血压可能表现为孤立的肺动脉高血压(PAH,在SSc中患病率为13-35%)或因左心室参与/舒失功能障碍或ILD/低氧血症7引起的肺高血压。SSc-ILD 患者和 SSc-PAH 患者之间的抗体特征不同。例如,抗Scl-70抗体的存在与SSc-ILD8有关,而抗中心抗体在PAH患者中比没有PAH9的患者更常见。
SSc-ILD的症状包括呼吸困难、咳嗽、胸痛和运动限制。ILD是SSc10、11、1211,12发病率的主要原因。10因此,据报告,SSc-ILD患者每年的全因医疗费用高于没有ILD的患者:31,285-55,446美元,而18,513-23,268美元,分别为13美元。
SSc-ILD是SSc患者死亡的主要原因,占这组10,14,14例死亡的30-35%。据报告,SSc-ILD患者的中位存活率为5-8年10年、15年;相比之下,大约76%的SSc总人口从发病16年起存活超过10年。SSc-ILD中死亡的重要预测变量包括年龄、强制生命容量(FVC)、肺一氧化碳(DLCO)的基线扩散能力、高分辨率计算机断层扫描(HRCT)的疾病程度、肺高血压的存在以及Kerbs von den Lungren 6(KL-6)抗原17、18。17,

Protocol
1. HRCT 扫描
- 对胸部进行体积HRCT采集扫描36。.对比剂不需要36,37。36,
- 使用表 136、,37所示的参数获取以下采集。
- 获得从肺皮座到肺底的吸气扫描(体积)。
- 从肺皮座以下2厘米到肺底,获得一个苏皮的呼气性光晕扫描(连续10~20毫米的间隙)。
- 从卡米纳到肺碱,获得容易吸气的可选(连续10~20毫米的间隙)。

Representative Results
诊断
HRCT 上 SSc-ILD 的主要特征通常包括非特异性间肺炎 (NSIP) 模式,具有外围地面玻璃不常见和广泛的牵引支气管扩张(图1和图 2)。地面玻璃不一视其数具有广泛的病因,通常非特异性40、41、42。40,41,42具有辅助备用的中央优势或外围分布是 NSIP 的高度暗示(图3)。
通常,HRCT 图像中的 ILD 模式包括具有结构变形的隔离,导致牵引支气管扩张/支气管扩张(与 NSIP 的纤维化形式一致)。事实上,牵引支气管扩张和牵引支气管支气管炎往往是SSc-ILD的主要特征(图4)43。43其他发现可能包括蜂窝(图5;在有限形式的SSc中更常见)、球形隔膜增稠和球内线,以及微结核40,44。,44蜂巢是指通常直径(±3~10毫米)的聚集囊状空气空间,其壁厚、定义明确31。蜂窝和牵引支气管扩张是HRCT上常见的间质性肺炎(UIP)的主要特征。虽然这种模式最常见的与特发性肺纤维化(IPF)相关,原型纤维化ILD与渐进型表型,它有时可以看到在SSc-ILD10患者。最近,在与结缔组织疾病相关的ILD(包括SSc-ILD)和HRCT上的UIP模式的患者中发现了几个迹象,但在有IPF的患者中则没有发现。这些是直边符号(即,纤维化与肺碱的隔离,在颅骨平面上具有清晰的分界,在日冕图像上没有沿着肺的侧边边缘进行实质性延伸)、蜂窝主要(或旺盛的)符号(或旺盛的)符号(肺纤维部分的>70%),以及前上瓣符号(即, 上叶前部纤维化的浓度,与上叶其他方面的相对备用,并伴随下叶参与)45。直边符号也与NSIP病理学46有关,这是SSc-ILD10中的主要CT模式。
在SSc患者(图6)47、48、49和SSc-ILD47,48,49 47,48,48患者中,经常观察到有空气充沛的食道。虽然没有公认的高龄限制,扩张性食道可能不再有助于区分SSc-ILD和IPF,但由于食管运动障碍的发生率增加,扩张性食道在65岁以上的患者中可能更难解释。中位淋巴曲病(通常反应性),其中淋巴结的短轴超过10毫米,也经常观察到在SSc-ILD47,50,50的患者。肺动脉大小大于相邻的上升主动脉建议共存肺高血压(图6),即使在没有纤维化肺病的患者51,52,53。51,52,53巩固区域表明叠加感染、吸入、组织肺炎、出血或恶性肿瘤。由于SSc-ILD7中肺癌风险增加,必须监测结核;最常见的原发性癌症出现在SSc-ILD患者是腺癌7,7,54。
SSc-ILD 与 IPF15、,55共享许多临床、机械和病理相似性。然而,一些辐射功能允许分化这两个ILD15,45。,45在SSc-ILD中,与IPF相比,地面玻璃不全的比例更高,纤维化则不那么粗糙。在SSc的UIP的情况下,蜂巢可以在超过70%的纤维肺组织-旺盛的蜂窝符号56,57。56,此外,四角符号(也称为前额叶符号)在 SSc-ILD 中比 IPF 中更常见;这是一种炎症和/或纤维化的焦点或不成比例地涉及双边前侧上额叶和后上额下叶58的模式。
胸部放射图最初可能检测到 ILD;然而,它们不能提供足够的对比度分辨率来可靠诊断。在来自SSc-ILD患者的胸放射图中,最常见的模式是基础主导性视网膜59。其他功能可能包括可见的支气管扩张、体积损失和蜂窝化。与HRCT一样,扩张性充满空气的食道的存在可能有助于支持SSc-ILD47的诊断。
预后
几种不同的成像发现已经证明与SSc-ILD的预后有关。据报告,在HRCT疾病程度至少为20%的患者中,死亡率较高(10年生存率分别为43%对67%,疾病范围高于20%的患者中为60.同样,HRCT的高纤维化评分(基于视网膜和蜂窝化的程度)也与死亡率增加有关61。大食管直径与ILD严重性增加和DLCO48减少有关。肺密度和肺动脉直径可能被用来预测肺高血压的风险62。计算机化、定量CT参数也可以识别患者肺功能下降或死亡的风险。一项研究表明,从HRCT量化的ILD的程度可以用来预测FVC在12个月63年的下降。在另一项研究中,定量胸腔CT参数提供的死亡率风险结果与临床预测模型64一致。尽管基于成像的生物标志物具有明显的潜力,但目前最好在人群一级考虑,因为它们在单个患者中的临床效用尚未确定。
治疗反应
环磷酰胺和霉菌在SSc-ILD患者中提供适度的益处。在具有里程碑意义的斯氯德米尔马肺研究I,环磷酰胺治疗导致纤维化的进展较慢相比安慰剂65。最近,Scleroderma肺研究II报告与环磷酰胺66相比,与霉菌素软骨具有类似的疗效和增强的耐性。然而,仍然需要为SSc-ILD患者改进治疗方案。目前正在研究的疗法包括单克隆抗体(如利妥昔单抗、阿比妥祖马布)、抗纤维化剂(如宁达尼布、皮尔费尼酮)、直接血栓抑制剂dabigatran、蛋白酶抑制剂Bortezomib和造血干细胞移植19、67。19,

Discussion
虽然HRCT目前是诊断和评估SSc-ILD的决定性成像方法,但它使用电离辐射,而且相对昂贵。胸部放射线图可以代替进行,尽管这些不能促进与HRCT相同的鉴别诊断,而正常的胸辐射图并不能消除ILD的可能性。也许最好使用胸腔放射图是在HRCT扫描和排除复杂疾病(如传染性肺炎)之间监测渐进性疾病,从而在症状的急性恶化环境中。
HRCT 的一个感知限制是辐射照射。如前所述,进行CT扫描的新方法可使辐射暴露减少31,此外,目前的CT扫描仪提供了一系列先进技术,为未来降低近胸部放射成像水平的辐射暴露提供了可能性。或者,成像方法,如核磁共振成像或肺超声,可能被用来避免病人在未来32,71,72,7371,72,73暴露辐射。32我们相信,虽然与成像利用相关的风险收益注意事项,CT 在诊断和患者管理方面的优势远远大于潜在风险。
成像数据,特别是 HRCT,提供了最重要的信息,使 SSc-ILD 的诊断工作得以进行。详细考虑HRCT扫描的模式和特征通常足以区分SSc-ILD与其他肺部疾病,从而避免了对侵入性活检手术的需要。
