Vein Graft Interposition

Vein graft interposition is a surgical technique that restores continuity in a damaged or missing blood vessel by placing a segment of vein between the vessel ends. The graft acts as a conduit and is secured with vascular anastomoses, allowing blood to flow across a gap that cannot be closed directly without excessive tension; the vein may be harvested from the patient or obtained from another source. This approach is used in vascular reconstruction after trauma, tumor resection, or complex vessel repair, helping reestablish perfusion and preserve downstream tissue. Graft patency and healing depend on appropriate sizing, blood flow, and meticulous surgical technique.

Neointimal hyperplasia is the primary cause of stenosis in arterialized veins. We propose a new protocol whereby the right external jugular vein is grafted using the cuff technique in the common carotid artery of Sprague Dawley rats. The survival rate was 100 % at the time point of sacrifice.

To improve our knowledge of cellular and molecular neotissue formation, a murine model of the TEVG was recently developed. The grafts were implanted as infrarenal vena cava interposition grafts in C57BL/6 mice. This model achieves similar results to those achieved in our clinical investigation, but over a far shortened time-course.

This method describes the placement of interposition vein grafts in rabbits, the transduction of the grafts, and the achievement of durable transgene expression. This allows the investigation of physiological and pathological roles of transgenes and their protein products in grafted veins, and testing of gene therapies for vein graft disease.

Carotid Artery and Jugular Vein Interposition: A Technique to Induce Venous Intimal Hyperplasia in Rabbit Model

This video demonstrates the interposition of the rabbit carotid artery with the jugular vein to induce venous intimal hyperplasia by subjecting the veins to arterial pressure. This model will help investigate molecular and genetic interventions to attenuate the progression of intimal hyperplasia after revascularization surgeries.
