Trigeminal Nerve Injury

Trigeminal nerve injury is damage to the fifth cranial nerve, which carries facial sensation and controls the muscles of mastication, and it can disrupt pain, temperature, touch, and oral function. Injury may arise from trauma, surgery, compression, inflammation, or disease, and abnormal signaling from damaged or regenerating sensory fibers can produce numbness, tingling, or neuropathic facial pain. Neuroscience research uses clinical examination, sensory testing, and neuroimaging to characterize these changes and distinguish peripheral nerve damage from altered central pain processing. Understanding the injury supports diagnosis, symptom management, nerve repair strategies, and research into how sensory circuits recover or become chronically sensitized.

2017

Treating chronic or neuropathic facial pain can be challenging when medical or standard treatment fails. Subcutaneous nerve field stimulation is the least invasive form of neuromodulation and is used for chronic back pain. We applied this technology to treat chronic and neuropathic trigeminal facial pain.

This manuscript describes a rodent model to study trigeminal neuropathic pain. The methods described include surgical procedures to perform a chronic constriction injury of the rat’s infraorbital nerve and the post-surgical behavioral tests to measure the changes in spontaneous and evoked behavior that are indicative of persistent pain and allodynia.

Subcutaneous Trigeminal Nerve Stimulation to Relieve Neuropathic Facial Pain

Source: Jakobs, M., et al. Subcutaneous Trigeminal Nerve Field Stimulation for Refractory Facial Pain. J. Vis. Exp. (2017)This procedure demonstrates the implantation of subcutaneous electrodes for trigeminal nerve stimulation to treat neuropathic facial pain. The electrodes are tunneled subcutaneously from the affected facial region to an infraclavicular pocket, where they are connected to an internal pulse generator (IPG). The IPG delivers electrical stimulation, activating mechanoreceptor...

Source: Svientek, S. R., et al. Fabrication of the Composite Regenerative Peripheral Nerve Interface (C-RPNI) in the Adult Rat. J. Vis. Exp. (2020).This video demonstrates the surgical construction of a composite regenerative peripheral nerve interface (C-RPNI) in a rat model of peripheral nerve injury. The procedure involves attaching a transected peroneal nerve to a muscle graft and overlaying it with a dermal skin graft to promote nerve regeneration. Over time, motor and sensory neurons...
