肺组织中循环肿瘤细胞可视化肺定植检测方法的建立

需要一种动物模型来解析循环肿瘤细胞(CTCs)在癌症转移过程中促进肺部定植的作用。在此,我们建立并成功实施了一种体内检测方法,专门用于验证CTC表面聚合纤连蛋白(polyFN)组装对肺部定植的必要性。

转移是导致癌症死亡的主要原因。循环肿瘤细胞(CTCs)在促进癌症转移中的作用已被广泛研究,其中CTCs在肺部的定植对早期肺转移过程具有关键贡献。因此,动物模型是唯一能够完整模拟转移全身性过程的研究手段。鉴于既往用于研究CTCs在血管外渗中作用的实验设计存在诸多问题,我们建立了一种 体内 肺定植实验,采用长期荧光细胞示踪剂羧基荧光素琥珀酰亚胺酯(CFSE)标记悬浮肿瘤细胞,并在切除肺组织前进行肺灌注以清除非特异性滞留的循环肿瘤细胞(CTCs),随后进行共聚焦成像与定量分析。研究发现,CTC表面组装的聚合纤连蛋白(polyFN)在转移性肿瘤组织的最终建立过程中介导了肺定植。在此,为特异性验证CTCs表面polyFN组装在肺定植及血管外渗过程中的必要性,我们开展了短期肺定植实验,将稳定表达FN-shRNA(shFN)或对照scramble-shRNA(shScr)、并预先用20 μM CFSE标记的悬浮Lewis肺癌细胞(LLCs)经尾静脉接种至C57BL/6小鼠体内。我们成功证明,与shScr LLC细胞相比,shFN LLC细胞在小鼠肺部的定植能力显著降低。因此,该短期实验方法可广泛应用于特异性验证循环中CTCs的肺定植能力。

转移是导致癌症死亡的主要原因1,2。来源于原发组织的肿瘤细胞以悬浮状态进入循环系统,并在定植远端器官之前,需耐受多种血源性挑战,例如,失巢凋亡、免疫攻击以及由血压或几何限制引起的剪切应力所造成的损伤,而远端器官定植是决定转移成功的关键步骤3,4,5,6。因此,目前人们正大力开展循环肿瘤细胞(CTCs)的特征分析,并将这些特征与肿瘤恶性程度、转移能力及癌症患者的生存率相关联7,8,9。由于癌症转移过程特指一种体内事件,动物模型是唯一能够完整再现转移系统性全过程的研究手段10,11,12

循环肿瘤细胞(CTCs)通过多个细胞事件形成转移性肿瘤组织,包括对远端器官的定植1,2。然而,目前最常用的转移检测方法13,14,15无法实现对CTC定植远端器官过程的直接观察。因此,迫切需要建立一种用于可视化CTC定植过程的in vivo检测方法。尽管已有若干in vivoex vivo短期肺部定植检测方法被设计出来,但仍存在诸多问题和局限性。例如,虽然在这些检测中已使用过表达绿色荧光蛋白(GFP)的肿瘤细胞22,23,但通过显微镜观察获得具有足够GFP荧光强度的稳定转染和克隆肿瘤细胞需要较长时间。类似地,尽管已采用长期细胞示踪剂CFSE对肿瘤细胞进行瞬时标记以替代表达GFP的肿瘤细胞,但仍难以判断CFSE标记的肿瘤细胞是真正附着于组织还是仅仅存在于切除远端器官的血管腔内16,17

在循环肿瘤细胞(CTCs)表面组装的纤维连接蛋白多聚体(polyFN)已被发现对转移性肿瘤组织的最终形成具有关键作用18,19,20,21,22。本研究中,我们进行了短期肺定植实验,将稳定表达FN-shRNA(shFN)或对照shRNA(shScr)并预先用CFSE标记的悬浮型Lewis肺癌细胞(LLCs)通过尾静脉接种至C57BL/6小鼠体内。接种后2–3天,先用磷酸盐缓冲液(PBS)灌注小鼠肺组织,以彻底清除血管内未黏附的CTCs,随后进行共聚焦显微镜观察并对肺部定植的LLCs进行定量分析。我们明确显示,与shScr LLCs相比,shFN LLCs在肺部的定植数量及肿瘤结节数量均显著减少,从而有力证实了CTCs表面polyFN的组装在促进其肺部定植与生长过程中的重要作用。本研究提示polyFN在癌症转移中的功能值得进一步深入探讨。

方案

所有小鼠实验均按照本机构的指南(《实验动物护理和使用指南》,成功大学医学院)进行。

1. 仪器、培养基和培养皿的准备

  1. 开始实验操作前,准备无菌外科缝线、1 mL 注射器配 26G/0.5 cm 针头(用于尾静脉注射)、3 mL 注射器配 24G/3 cm 针头(用于肺灌注)、杜氏改良伊格尔培养基(DMEM)、胰蛋白酶-EDTA 溶液(5 g/L 胰蛋白酶和 0.53 mM EDTA)、胎牛血清(FBS)、1× 磷酸盐缓冲液(PBS)(137 mM NaCl、2.7 mM KCl、10 mM Na2PHO4 和 1.8 mM KH2PO4),以及 6 cm 培养皿。

2. 悬浮肿瘤细胞的制备与回收

  1. 准备无菌的 Dulbecco 改良 Eagle 培养基(DMEM),含 10% 或 20% 胎牛血清(FBS),并新鲜添加 2 mM L-谷氨酰胺、1× PBS 和 0.05% 胰蛋白酶-EDTA。
  2. 在含 10% FBS 的 DMEM 培养基中,于 37 °C 培养细胞,直至培养皿中细胞融合度达到 70–80%。
  3. 从培养皿中移除培养基(DMEM),随后用 2 mL 无菌 1× PBS 洗涤两次。
  4. 向培养皿中加入 1 mL 0.05% 胰蛋白酶-EDTA,充分覆盖所有细胞。立即吸出 800 μL 溶液,仅保留 200 μL 在培养皿中。于 37 °C 孵育 30 秒至 1 分钟(根据细胞类型而定),直至大部分细胞呈悬浮状态。
  5. 直接向培养皿中加入 1 mL 含 10% FBS 的新鲜 DMEM,以终止胰蛋白酶的酶活性,并使用 1000 µL 移液器剧烈吹打细胞悬液数次,制备单细胞悬液。
  6. 将细胞从培养皿转移至 1.5 mL 微量离心管中,在室温下以 162 × g 离心 3 分钟,沉淀细胞。
  7. 吸除上清液,将肿瘤细胞重悬于 1.5 mL 含 20% FBS 的 DMEM 中,并于 37 °C 以翻转混匀方式悬浮培养 2 小时。
    注意:若恢复期间培养基变黄,应更换为含 20% FBS 的新鲜培养基。

3. 使用羧基荧光素琥珀酰亚胺酯(CFSE)对悬浮肿瘤细胞进行荧光染色与分析

  1. 细胞悬浮复苏后,使用台盼蓝在血细胞计数板中计数适量的活细胞数量,用于滴定荧光染色剂量及尾静脉注射。
  2. 在室温下以162 × g离心3分钟沉淀细胞,将细胞沉淀重悬于1 mL无菌1×PBS中,随后在室温下以162 × g再次离心3分钟。
  3. 将细胞沉淀重悬于500 µL含10%胎牛血清(FBS)和0、5、10、20或40 µM CFSE的1×PBS中进行剂量滴定,并在37 oC避光孵育10分钟。
  4. 在室温下以162 × g离心3分钟沉淀细胞,将细胞沉淀用4 mL含1% FBS的DMEM重悬,重复此洗涤步骤三次。最后一次洗涤时,仅用4 mL DMEM重悬细胞沉淀。
  5. 对CFSE标记的细胞进行荧光激活细胞分选(FACS)分析,以定量单个细胞的荧光强度,或进行尾静脉注射。
    注意:在进行FACS分析或尾静脉注射前,需将细胞置于冰上保存。

4. 肺部定植实验

  1. 准备用 20 µM CFSE 标记的细胞(参见步骤 3.1 至 3.4)。
    注意:根据流式细胞术分析的滴定结果(参见步骤 3.3),确定用于标记肿瘤细胞的 CFSE 工作浓度。
  2. 使用卤素灯逐只加热 4–6 周龄雄性 C57BL/6 小鼠(共 6 只)的尾巴 5–10 分钟,以扩张尾静脉。
  3. 用 1 mL(26Gx1/2)注射器充分混匀并吸取 CFSE 标记的肿瘤细胞,避免细胞悬液中产生气泡(最终细胞密度为 5 × 106 个细胞/mL)。
  4. 将 200 µL DMEM 中含有的 1 × 106 个 CFSE 标记的肿瘤细胞小心注入固定在小鼠固定器中的小鼠尾静脉。
    注意:注射过程中,若针头准确插入尾静脉管腔内,肿瘤细胞应能顺利进入循环系统,无需用力推注。若推注困难,则很可能已将细胞注入尾静脉外的皮下组织。
  5. 将静脉注射了 CFSE 标记肿瘤细胞的小鼠分为两组:一组用于肺灌注后进行共聚焦显微镜观察及 ImageJ 肺定植分析;另一组则在长期肺定植实验中继续饲养 4–5 周。
  6. 在时间进程和剂量依赖性滴定实验中,于静脉注射 1 × 106 个肿瘤细胞后的 20、38 或 45 小时,使用 50–75 mg/kg 的 Zoletil 50 对荷瘤小鼠进行麻醉,该麻醉剂已被广泛接受并适用于此类实验23
    注意:麻醉期间需在小鼠眼部涂抹兽用眼膏,以防角膜干燥。
  7. 使用手术剪从腹部至胸部纵向切开皮肤和皮下组织。用手术剪和镊子打开胸膜腔,充分暴露心脏和肺组织。
    注意:操作时应小心避免损伤血管。
  8. 用无菌手术缝线结扎上腔静脉(SVC)和下腔静脉(IVC),以防止肺灌注过程中灌注液反流。
  9. 用手术剪剪开左心室壁,制造一个约 2–4 mm 的切口,以便灌注液从肺部流出。
  10. 用 3 mL 注射器将 1× PBS 注入右心室,同时在肺灌注过程中持续吸引排出液体,直至肺组织颜色由红色变为完全苍白。

5. 肺定植肿瘤细胞的共聚焦显微镜成像与分析

  1. 用手术剪和镊子从胸膜腔中取出已呈苍白的肺组织,并剪除气管和结缔组织韧带,分离出五个肺叶24
  2. 图3B所示,将尼龙缝合线缠绕在两根金属棒上,制成类似担架的肺组织支撑架,金属棒之间留有空隙用于放置肺叶,并在表面形成网状结构。将肺组织支撑架置于一个6 cm的培养皿中。
    注意:该支撑架将用于在共聚焦显微镜物镜下固定肺叶。
  3. 将肺叶嵌入并固定在肺组织支撑架的网状结构上,用1×PBS覆盖并保持肺叶湿润。
  4. 将放置有肺叶及支撑架的6 cm培养皿置于共聚焦显微镜下,采集记录肺部定植肿瘤细胞的荧光图像。
  5. 成像时使用物镜(5X,MPLN,数值孔径:0.1,工作距离:20,空气介质)。
  6. 将滤光轮切换至NIBA(激发/发射波长:470–495 nm/510–550 nm),使用488 nm激光激发CFSE,以清晰观察肺叶中荧光点状信号的图像。
  7. 将滤光轮切换至R690(适用于MaiTai HP DeepSee激光器),以2.0 µs/像素的扫描速度进行扫描,并优化高压(HV)、增益(Gain)和偏移(offset)参数。
  8. 使用显微镜软件以10 µs/像素的扫描速度采集荧光图像(512 × 512像素)。
  9. 使用ImageJ和GraphPad软件对显微图像进行定量分析和统计分析,方法如前所述21

6. 长期肺定植实验

注意:重复步骤 4.1 至 4.5。

  1. 在肿瘤细胞接种后4-5周处死小鼠,并用Bouin's Fluid25固定其肺组织1至2天。
  2. 使用软件21对小鼠肺组织中的肿瘤结节数量进行定量和统计分析。

在执行之前 体内 如图所示的肺部定植实验 图1A 为了检测悬浮肿瘤细胞上的多聚纤维连接蛋白(polyFN)是否在促进转移过程中介导肺定植和/或外渗,我们首先滴定不同浓度的CFSE(一种可穿透细胞并共价结合细胞内含胺分子的荧光化合物)26,27 在悬浮的肿瘤细胞中,CFSE 能有效标记肿瘤细胞,并在细胞内保持较长时间。我们发现,CFSE 对肿瘤细胞的标记效果呈剂量依赖性,如图所示。 图1B. 6 x10 的 CFSE 标记5 每毫升LLC细胞数在20 µM时几乎达到平台期,如图所示 图1C.
