通过盲肠斑块精确定位建立小鼠慢性结肠炎相关结直肠癌的阑尾切除模型

本方案描述了一种简便的小鼠阑尾(盲肠斑)切除手术,随后诱导与炎症性肠病相关的结直肠癌。该小鼠阑尾切除模型可用于研究阑尾在人类胃肠道疾病发病机制中的生物学作用。

摘要

人类阑尾最近被发现与多种复杂疾病的发病机制具有重要的生物学关联,例如结直肠癌、炎症性肠病和帕金森病。为了研究阑尾的功能,目前已建立了一种与肠道疾病相关的实验小鼠阑尾切除模型,本文详细描述了该模型的逐步操作流程。本报告介绍了一种简便的小鼠盲肠斑块切除方法,随后通过葡聚糖硫酸钠(DSS)与氧化偶氮甲烷(AOM)联合使用,化学诱导慢性结肠炎相关性结直肠癌的发生。在雄性C57BL/6小鼠中,与假手术组相比,切除盲肠斑块后,特异性IgA细胞数量及IgA浓度均显著降低 。同时给予2% DSS和AOM后,假手术组和阑尾切除组小鼠的存活率均接近80%,且未出现显著的体重下降。组织学结果证实了结肠炎症的存在以及不同程度的腺癌病变。该模型可用于研究阑尾在维持肠道微生物群稳态、肠道结肠炎及恶性肿瘤发病机制中的功能作用,同时也可用于靶向药物治疗的潜在开发。

引言

阑尾切除术是一种标准的临床外科手术,通常因阑尾发炎(例如阑尾炎)而将其切除1,2,3。然而,人类阑尾的生物学功能仍存在争议4,5,6。长期以来,阑尾被视为从大肠盲肠延伸出的退化残留结构。直到最近,进化、免疫学、形态学和微生物学研究提示,阑尾可能具有特定功能,包括产生免疫球蛋白(如IgA和IgG)、多种对肠相关淋巴组织(GALTs)中适应性免疫应答至关重要的B细胞和T细胞,以及为大肠补充共生微生物群落6,7,8,9,10,11,12

对既往接受过阑尾切除术或急性阑尾炎患者的临床流行病学研究也揭示了阑尾在人类疾病发病机制中的潜在作用,例如炎症性肠病(IBD)、结直肠癌以及非胃肠道疾病(如帕金森病和心血管疾病)13,14,15,16,17,18。例如,一项近期针对75,979名阑尾切除术患者的大型亚洲人群队列研究显示,阑尾切除术与后续结直肠癌的发生之间存在显著关联,而结直肠癌是发病率和死亡率均较高的最常见恶性肿瘤之一14,19。因此,建立一种与人类情况相似的合适动物阑尾切除模型,将有助于研究阑尾在疾病发生发展过程中的生物学功能及分子机制。

许多哺乳动物具有阑尾或类似阑尾的器官,包括灵长类、兔形目动物(例如兔子)、某些啮齿类动物以及有袋类动物20。在体型较小且常用的实验动物中,兔子的蚓突在形态上与人类阑尾相似21,22,但与人类相比,兔子的肠道相关淋巴组织(GALT)极为发达,因为大部分淋巴组织也存在于位于小肠和大肠的派伊尔结(Peyer’s patches)中21。此外,兔子在淋巴滤泡结构、T细胞分布以及免疫球蛋白密度方面均与人类存在差异,这使得将其阑尾作为研究对象并不合适21

小鼠是研究人类病理生理学以及测试现有和新型治疗方法最常用的动物模型23,24,25。小鼠盲肠尖端存在一个单一的白色大型淋巴样细胞簇,称为盲肠斑(caecal patch),被认为具有类似于人类阑尾的功能26,27,28。然而,在小鼠中将盲肠斑与盲肠分离在实际操作上非常困难。迄今为止,用于在小鼠模型中诱导阑尾炎的常见外科手术方法均需通过腹壁做相对较大的切口(例如,1–2 cm),以暴露整个盲肠(补充表129,30,31,32,33,34,35,36

本文介绍了一种简便的小鼠盲肠组织切除手术方案,以建立与胃肠道疾病相关的阑尾切除模型。随后联合使用基因毒性试剂AOM和促炎试剂DSS,诱导产生类似于人类的结肠炎相关性结直肠癌。已有研究表明,炎症性肠病(IBD)是肠道癌症的危险因素37,38。AOM/DSS联合诱导慢性结肠炎相关性结直肠癌的方法已被广泛验证,详细操作步骤可参考Neufert等和Thaker等的研究39,40。该小鼠阑尾切除模型具有可重复性和高效性,可用于研究阑尾调控的肠道炎症及结肠微生物群,尤其适用于炎症性肠病(IBD)和结直肠癌发生与发展的机制研究。

方案

所有动物实验程序均经西安交通大学实验动物福利与使用委员会批准(编号:XJTULAC2019-1023)。

1. 小鼠阑尾切除术

  1. 将8至10周龄的C57BL/6雄性小鼠饲养于经认证的无特定病原体(SPF)环境中,术前适应1周。
  2. 准备以下无菌手术器械:显微剪刀1把、显微镊子1把、两种规格(4-0和8-0)的灭菌不可吸收缝线、带针持的电凝笔、75%医用酒精、碘类消毒液(如安尔碘),以及一包无菌棉签。
  3. 用10 mL注射器抽取预热的0.9%生理盐水,用于术中腹腔冲洗和补液。
  4. 对未禁食的小鼠按100 mg/kg剂量经腹腔(i.p.)注射1%戊巴比妥钠进行麻醉,通过检查足底反射是否消失来判断麻醉深度。
    注意:单次给药以确保小鼠达到充分镇静效果。若手术时间较短,经腹腔注射50 mg/kg戊巴比妥钠可能已足够。
  5. 使用电动剃毛器轻柔剃除腹部毛发。
  6. 将小鼠置于加热垫上,防止术中低体温。
  7. 将小鼠仰卧位固定于操作平台,用四条医用胶带分别固定四肢,防止术中体位移动。
  8. 轻触整个腹部,寻找局部隆起感。
    注意:通常该隆起感即为盲肠的准确位置。为避免损伤其余肠段,切开腹腔前预先定位盲肠至关重要。
  9. 覆盖无菌洞巾,使用碘类消毒液与75%酒精交替擦拭剃毛区域进行皮肤消毒,重复2次。
  10. 于腹部正中线做一长度为0.5–1.0 cm的纵行切口。
  11. 根据预先确定的盲肠位置,找到盲肠并轻柔外翻约1 cm,以识别盲肠斑(caecal patch)。使用无菌预热、pH值正常的生理盐水润湿肠道。
    注意:小鼠的盲肠斑位于盲肠表面,特征为存在白色卵圆形淋巴滤泡。
  12. 为预防术后出血和感染等潜在并发症,使用8-0缝线结扎盲肠系膜血管。用无菌生理盐水润湿盲肠。
  13. 在靠近盲肠顶端与近端结肠交界处,用4-0缝线做一个开放环状标记,以界定切除范围。
    注意:以开放环标记盲肠还可防止切割后内容物泄漏。
  14. 用显微剪刀在标记的切除线以下切除盲肠斑,并用医用棉签擦净残留的盲肠内容物,随后用碘类消毒液对盲肠残端进行消毒。
  15. 使用8-0缝线连续缝合关闭盲肠残端。
  16. 仔细移除此前用于标记切除范围的4-0缝线环。
  17. 用碘类消毒液对缝合部位进行消毒,再次用生理盐水润湿手术区域。
  18. 使用8-0缝线连续缝合关闭肌层。
  19. 使用4-0缝线间断缝合关闭皮肤层。
  20. 用碘类消毒液对切口消毒2次,再用75%医用酒精去除残留碘液。
  21. 轻柔将小鼠翻回俯卧位,皮下(s.c.)注射0.1 mg/kg体重的布托啡诺及0.4 mL生理盐水,随后将小鼠置于加热垫上直至苏醒。
  22. 将小鼠放回无菌笼具中,术后密切观察3天,注意疼痛迹象以确保顺利恢复。
    注意:术后可根据需要给予0.05 mg/kg布托啡诺以缓解个体小鼠疼痛。手术后让小鼠恢复7天,以便后续诱导结肠炎相关性结直肠癌模型。

2. 使用AOM和DSS诱导慢性结肠炎相关性结直肠癌

注意:在阑尾切除术后第7天进行此操作。

  1. 将25 mg AOM溶解于2.5 mL 0.9%无菌生理盐水中,配制成浓度为10 mg/mL的AOM储备液。
    注意:AOM 对光敏感。
    1. 将配制好的AOM储备液每2.5 mL分装至用铝箔包裹的5 mL玻璃管中,每次使用后于-20 ˚C保存。
    2. 将一份数量的AOM解冻一次,并用0.9%无菌生理盐水按1:10比例稀释至浓度为1 mg/mL。
      警告:AOM 具有极强的致癌性,因此整个配制过程必须在通风橱中进行。
  2. 将4 g DSS粉末溶于200 mL高压灭菌水中,配制2%(w/v)DSS溶液。
    注意:DSS 的浓度可能因小鼠品系、性别和诱导周期而异;其他小鼠品系可使用 3% DSS。
  3. 同时向小鼠给予新鲜配制的AOM/DSS。操作步骤如下。
    1. 按每克体重0.01 mL的剂量,向每只小鼠腹腔注射 freshly prepared AOM working solution,并将高压灭菌水替换为2% DSS溶液,持续5天 随意摄取
      注意:每笼饲养五只小鼠;定期检查DSS饮水瓶,确保在处理期间无沉淀产生。
    2. 每天称重并密切观察每只小鼠。
      注意:在实验过程中,若小鼠体重减轻超过初始体重的20%,或出现蜷缩、眯眼、低体温及活动能力下降等症状,应实施安乐死。
  4. 在给予AOM/DSS后的第6天至第21天期间,每天提供一瓶新鲜的高压灭菌水图1B).
    注意:这是一个包含21天的完整周期。
  5. 重复步骤 2.3–2.4 两个循环,于第 70 天处死小鼠。

3. 结肠炎症与肿瘤的评估(AOM 给药后第70天)

  1. 将小鼠置于安乐死舱内,以10%的充气速率吸入CO2进行处死,随后行颈椎脱位确认死亡。
  2. 从回盲部上方至肛门处完整取出整个结肠。
  3. 沿结肠纵轴切开,暴露其管腔面。将整段结肠切成约10 cm长的片段,并将结肠组织置于10%福尔马林中固定72小时,用于苏木精-伊红(H&E)染色。

结果

小鼠阑尾切除模型的建立

通过遵循如图1所示的连续外科手术和诱导步骤,可建立该与慢性结肠炎相关的结直肠癌小鼠阑尾切除模型。盲肠最常见的位置位于左、右髂窝,其次为腹中线位置(图2)。在开腹切口前,通过触诊法对盲肠进行术前定位的成功率接近70%(补充信息,表S2)。阑尾切除手术主要包含六个关键步骤(图3)。在阑尾切除术中,沿腹中线做长度仅为0.5–1.0 cm的小切口,对于最大限度减少手术创伤至关重要(图3A)。若发生腹腔内渗漏,需结扎肠系膜血管并关闭盲肠残端,此时应采用连续缝合而非单纯结扎(图3B,C,D,E)。

讨论

采用具有高小鼠存活率的手术步骤,建立了用于结肠炎相关性结直肠癌研究的小鼠阑尾切除模型。大多数情况下,由于盲肠位于腹壁下方(补充表1、补充表2图2),在未进行剖腹探查前难以预先判断其确切位置。在本手术方案中,引入了一种简便的触诊隆起步骤,并提供了盲肠位置的定量评估方法,以指导更精确地术前定位盲肠(图2)。通过利用解剖学特征切除盲肠斑块,最大限度减少了对肠道非目标区域的干扰,从而降低了阑尾切除术后潜在并发症及腹腔感染的发生率。

该模型包含三个关键步骤。首先,在开腹手术前进行视诊和触诊有助于确定盲肠的大致位置。尽管盲肠最可能位于腹腔左髂区,但自盲肠顶端突出的盲肠斑块常呈游离状态(即可以移动或嵌入周围组织),更常发现于腹膜中段下方(图2)。其次,建议在腹部正中线做一小切口,并准确暴露盲肠斑块。这是因为腹部正中线存在一个较薄弱的区域,在此处切开可减少对腹壁肌肉层的潜在损伤。此...
