一种通过短暂双侧颈总动脉阻塞诱导的缺血性视网膜损伤小鼠模型

本文介绍了一种通过暂时性双侧颈总动脉结扎并结合夹闭法建立的小鼠视网膜缺血模型。该模型有助于理解由心血管异常引发的视网膜缺血的病理机制。

摘要

糖尿病性视网膜病变、视网膜动静脉阻塞以及眼缺血综合征等多种血管性疾病均可导致视网膜缺血。为研究视网膜缺血的病理机制,需要建立相应的实验模型。从解剖学角度看,主要的视网膜供血血管为眼动脉(OpA),而OpA起源于颈总动脉(CCA)的颈内动脉。因此,破坏CCA可有效引发视网膜缺血。本文通过短暂性双侧颈总动脉阻塞(tBCCAO)建立了一种小鼠视网膜缺血模型:用6-0丝线结扎右侧CCA,并使用夹子暂时阻塞左侧CCA 2秒,结果显示tBCCAO可诱导急性视网膜缺血,进而导致视网膜功能障碍。该方法仅需使用手术针和夹子,减少了对复杂手术器械的依赖;同时缩短了阻塞时间,以最大限度降低动物意外死亡率(这在大脑中动脉阻塞小鼠模型中较为常见),并保持了常见视网膜缺血表型的可重复性。该模型可用于研究小鼠缺血性视网膜病变的病理生理机制,并进一步用于体内药物筛选。

引言

视网膜是视觉功能的神经感觉组织。由于视觉功能需要大量氧气,视网膜被认为是体内耗氧量最高的组织之一1。由于氧气通过血管输送,视网膜容易受到血管性疾病的影响。多种类型的血管疾病,如糖尿病性视网膜病变和视网膜血管(静脉或动脉)阻塞,均可诱发视网膜缺血。为了研究视网膜缺血的病理机制,建立可重复且具有临床相关性的实验性视网膜缺血模型被认为是必要的。通过插入血管内缝线阻塞大脑中动脉(MCAO)是目前最常用的建立体内啮齿类动物实验性脑缺血模型的方法2,3。由于眼动脉(OpA)与大脑中动脉(MCA)位置邻近,MCAO模型也被同时用于研究视网膜缺血的病理生理机制4,5,6。为了同时诱导脑缺血和视网膜缺血,通常需要通过颈总动脉(CCA)或颈外动脉(ECA)切口插入较长的缝线。这些方法操作困难,完成手术所需时间较长(每只小鼠超过60分钟),且术后结果变异度较高7。因此,开发一种更优的模型以解决上述问题仍具有重要意义。

在本研究中,我们仅使用短暂的双侧颈总动脉夹闭(tBCCAO)方法,通过针头和动脉夹诱导小鼠视网膜缺血,并分析了视网膜缺血损伤的典型结果。在本视频中,我们将演示tBCCAO操作过程。

方案

本文所述所有方法均已获得庆应义塾大学医学院机构动物护理和使用委员会(IACUC)的批准。

1. 手术器械和动物的准备

  1. 将手术器械进行高压灭菌,并保存在70%乙醇中。每次新的手术操作前,需使用70%乙醇仔细清洁手术器械。
  2. 在特定病原体阴性(SPF)环境中准备雄性BALB/cAJc1小鼠(6周龄,26–28 g),以确保手术前后及手术过程中维持无菌条件。

2. 短暂性双侧颈总动脉阻断(tBCCAO)

  1. 通过腹腔注射给予小鼠麻醉剂混合物(midazolam 40 μg/100 μL、medetomidine 7.5 μg/100 μL 和 butorphanol tartrate 50 μg/100 μL),该混合物称为“MMB”,具体方法如前所述8,9。用手固定小鼠背部皮肤,防止其在完全麻醉前因活动而碰撞眼睛。
    1. 通过轻捏小鼠脚趾判断麻醉深度,直至小鼠无反应,此方法常用于评估是否达到完全麻醉状态10
      注意:通常小鼠在5分钟内即可入睡。不同机构可能采用不同的全身麻醉配方。
  2. 向眼睛滴加一滴0.1%的纯化透明质酸钠滴眼液,以防止麻醉期间角膜干燥。
  3. 将小鼠仰卧放置,并使用胶带固定其四肢。
  4. 手术前用70%乙醇对小鼠颈部区域进行消毒。
    注意:未额外剪除毛发,因为这可能导致后续皮肤炎症11,12
  5. 使用刀片沿颈部正中线进行矢状切口(图1)。
    注意:切口应位于颈部、胸骨与气管之间的中线上。
  6. 使用两把镊子小心分离双侧唾液腺,并将其移开,以便清晰暴露下方的颈总动脉(CCA)。
  7. 小心地将右侧颈总动脉(CCA)从相应的迷走神经及伴行静脉中分离出来,避免损伤这些结构,并在右侧CCA下方穿入两根6-0丝线。结扎两根线以完全阻断血流(图1)。
    注意:操作过程中可能会损伤小静脉。若出现出血,需擦拭干净以确保颈总动脉视野清晰。
  8. 小心地从左侧迷走神经及伴行静脉中分离出左侧颈总动脉(CCA),避免损伤周围结构,并使用动脉夹夹闭左侧CCA 2秒(图1)。
    注意:需预先将一根6-0丝线穿入左侧CCA下方,作为夹闭位置的标记。
  9. 松开动脉夹恢复左侧CCA血流后,使用6-0丝线缝合颈部切口,并在伤口处涂抹抗生素(50 μL)以预防细菌感染。
    注意:重新开放左侧CCA时,应轻柔地移除动脉夹,避免损伤动脉壁。
  10. 向小鼠腹腔注射0.75 mg/kg的阿替美唑盐酸盐,以促进其快速从深度麻醉中恢复。随后将小鼠放回垫有预热垫的饲养笼中。
    注意:在小鼠恢复足够意识、能够维持胸卧位之前,不得将其单独留下。
  11. 当小鼠苏醒后,注射0.4 mg/kg的butorphanol tartrate以进行镇痛管理。
    注意:此处可暂停实验流程。初步判断tBCCAO是否成功的一个提示是观察到小鼠出现眼睑下垂现象(图2)。
  12. 如需实施安乐死,可向小鼠注射3倍剂量的MMB混合液,随后处死并用于后续实验。

3. 一般观察(存活率和眼睑下垂)

  1. 术后检查第 0 天(术后)、第 1 天、第 3 天和第 7 天因各种原因导致的死亡率。
  2. 采用 4 分制评分法评估眼睑下垂程度:1 分 = 无下垂,2 分 = 轻度下垂(约 50%),3 分 = 重度下垂(超过 50%),4 分 = 重度下垂伴眼部分泌物。

4. 视网膜血流灌注

  1. 将200 μL的FITC-葡聚糖(25 mg/mL)注入小鼠左心室,该方法常用于观察小鼠视网膜血管中的血液灌注13,14
  2. 循环2分钟后摘除眼球,并用4%多聚甲醛固定1小时。按照先前描述的方法小心分离视网膜并进行平铺制片15,然后通过荧光显微镜观察。
  3. 以4倍放大倍数拍摄视网膜全层铺片图像,并使用先前描述的图像合并分析工具将其合并为单张图像16
  4. 利用NIH Fiji/ImageJ软件中的血管分析工具测量灌注区域面积。

5. 蛋白质印迹

  1. 在tBCCAO术后3小时和6小时,获取小鼠眼球并立即转移至含有冷PBS的培养皿中以分离视网膜。
  2. 视网膜分离后,按先前所述方法进行蛋白质印迹分析9
  3. 使用缺氧诱导因子-1α(HIF-1α;一种通用缺氧标志物)抗体和β-Actin(内参对照)抗体在4℃孵育过夜,随后孵育HRP标记的二抗,通过化学发光法显影信号。

6. 定量 PCR(qPCR)

  1. 在tBCCAO术后6、12和24小时,按照先前所述方法17对获取的视网膜组织进行qPCR处理。
  2. 使用实时PCR系统进行qPCR检测。所用引物列于表1中。采用ΔΔCT法计算不同转录本水平的倍数变化。

7. 免疫组织化学(IHC)

  1. 小鼠tBCCAO术后3天,取眼球并进行石蜡包埋。
  2. 使用切片机对石蜡包埋的眼球进行切片,获得眼组织切片。
  3. 对5 μm厚的石蜡切片进行脱蜡和染色,步骤同前文所述13
  4. 用胶质纤维酸性蛋白(GFAP;视网膜中星形胶质细胞和Müller细胞的可靠标志物)抗体4℃孵育过夜,随后孵育Alexa Fluor 555标记的二抗。
  5. 使用DAPI(4′,6-二脒基-2-苯基吲哚)对视网膜细胞核进行染色,并通过荧光显微镜观察信号。
  6. 采用4分制评分系统评估形态学变化,评分标准同前文所述13,18:0分=无信号,1分=节细胞层(GCL)中仅有少量阳性胶质终足,2分=少量标记的突起从GCL延伸至外核层(ONL),3分=大多数标记的突起从GCL延伸至ONL。

8. 视网膜电图(ERG)

  1. 在tBCCAO术后第3天和第7天,使用全视野刺激器(Ganzfeld dome)、信号采集系统和LED刺激器进行视网膜电图(ERG)检测,方法如前所述9
  2. 经过一夜暗适应后,在红光下使用MMB混合麻醉剂对小鼠进行麻醉。
  3. 使用0.5%托吡卡胺和0.5%去氧肾上腺素的混合溶液散大瞳孔。
  4. 将记录电极置于角膜接触镜上,参考电极置于口腔内。
  5. 采集每只动物双眼的ERG反应。
  6. 在暗适应条件下,使用不同强度的光刺激记录暗视ERG反应。
  7. 测量a波振幅,即从基线到a波最低点的垂直距离。
  8. 测量b波振幅,即从a波最低点到b波峰值的垂直距离。
  9. 实验过程中使用加热垫保持所有小鼠体温。

9. 光学相干断层扫描(OCT)

  1. 双侧颈总动脉永久性结扎(tBCCAO)术后2周,使用谱域光学相干断层扫描(SD-OCT)系统进行OCT检测,方法如前所述8,9
  2. 检测时,采用0.5%托吡卡胺与0.5%去氧肾上腺素混合溶液对小鼠进行散瞳处理,并使用MMB混合物进行全身麻醉。
  3. 从俯视扫描的赤道切面获取B扫描图像。
  4. 分别在视神经乳头0.2、0.4和0.6 mm处检查视网膜结构。
  5. 测量从视网膜神经纤维层(NFL)至外界膜(ELM)的视网膜厚度,并将各测量值的平均值作为该小鼠的视网膜厚度。
  6. 以蜘蛛图形式绘制结果。

结果

在 FITC-葡聚糖全身循环 2 分钟后,检测了假手术小鼠和 tBCCAO 手术小鼠左眼和右眼视网膜的血管情况(补充图 1)。在假手术小鼠的双侧视网膜以及 tBCCAO 手术小鼠的左眼视网膜中,FITC-葡聚糖清晰可见;而在 tBCCAO 手术小鼠的右眼视网膜中仅能部分检测到 FITC-葡聚糖。

tBCCAO术后,检测了眼睑下垂情况(图2)。右眼出现轻度(评分2分;75%)和重度眼睑下垂(评分3分和4分;25%),而左眼均无下垂现象(评分1分;93.75%),仅有一只小鼠出现轻度下垂(评分2分;6.25%)。尽管在tBCCAO手术小鼠中未明显观察到伴有眼部分泌物的严重眼睑下垂,但仍有一只小鼠表现出该表型(评分4分;6.25%)。

讨论

在本研究中,我们已证明使用简单的缝线和夹子进行tBCCAO可诱导视网膜缺血及伴随的视网膜功能障碍。此外,我们表明,与先前报道的视网膜缺血损伤模型构建方案相比,当前建立小鼠视网膜缺血模型的方案更为简便快捷2,3,7

从解剖学上讲,左右大脑动脉可通过后交通动脉(PCAs)相互连接,后者在威尔里斯环(circle of Willis)中提供侧支循环,以在单根血管发生闭塞或狭窄导致血流中断时,维持中枢神经系统的充分血液供应24,25图1)。Lee 等人证明,在 C57BL6 小鼠中,通过永久性单侧颈总动脉结扎(CCAO)可使视网膜血流灌注延迟约10分钟(并非同侧视网膜血流灌注完全阻断)13<...
