完全麻醉的A型血友病小鼠尾静脉横断出血模型

在麻醉小鼠中改良的尾静脉横断(TVT)出血模型是一种用于评估血友病出血的敏感体内方法。该优化后的TVT出血模型以失血量和出血时间作为终点指标,改进了其他模型,并避免将死亡作为终点指标。

摘要

尾部出血模型是血友病研究中的重要工具,尤其用于评估促凝血效应。由于尾静脉横断(TVT)存活模型对临床相关剂量的FVIII具有较高的敏感性,因此在许多研究中被优先采用;而其他已建立的模型(如尾夹模型)则需要更高剂量的促凝血化合物。为避免将存活作为实验终点,我们开发了一种TVT模型,以失血量和出血时间作为终点指标,并在整个实验过程中实施完全麻醉。简言之,将麻醉后的小鼠置于37°C温盐水中浸没尾部,并通过右侧尾静脉给予待测化合物。5分钟后,使用模板引导横断左侧尾静脉,随后将尾部放回盐水中,在接下来的40分钟内持续监测并记录所有出血事件,同时收集血液。若在损伤后10分钟、20分钟或30分钟时未见出血,则用湿润纱布轻轻擦拭切口两次以轻柔挑战血凝块。40分钟后,通过测定盐水中血红蛋白的含量来量化失血量。该方法快速且相对简便,可产生稳定且可重复的出血结果。与TVT存活模型相比,该方法更加人道,且不降低对药理干预的敏感性。此外,该模型可同时使用雄性和雌性动物,从而减少所需繁育的动物总数,符合3R原则。出血模型的一个潜在局限在于止血过程具有随机性,可能影响模型的可重复性。为克服这一问题,通过人工破坏血凝块确保在监测期间对凝块进行挑战,防止初级(血小板介导)止血过早终止出血。该模型的建立丰富了出血损伤模型的种类,为以标准化且人道的方式表征促凝血效应提供了新的选择。

引言

动物模型对于理解血友病的发病机制以及开发和测试治疗方案与疗法至关重要。凝血因子VIII基因敲除小鼠(F8-KO)是研究A型血友病广泛使用的模型1,2。这些小鼠重现了该疾病的关键特征,并已被广泛用于开发治疗方法,例如重组FVIII产品3,4,5和基因治疗策略6,7

有多种出血损伤模型可用于评估不同止血化合物在体内的药理作用。in vivo。其中一种凝血模型是小鼠尾静脉横断存活模型8,9,10,11,12,13,14,用于测量血友病小鼠在尾部横断后抵抗失血性死亡的存活能力。该方法在四十多年前被首次提出15,至今仍被使用9,16,17。然而,该模型以存活作为终点指标,需要对动物进行长达24小时的观察,在此期间动物保持清醒状态,因此可能经历疼痛和痛苦。

既往已有描述在全麻条件下持续时间较短的出血模型,例如尾尖剪切模型(也称为尾尖模型)8,18,19,20,21,22,23,24,25,26,27,28。然而,为了在出血刺激后实现血液丢失量的完全正常化,这些模型所需的促凝血药物剂量(例如FVIII)远高于临床实际给药剂量29。另一种在麻醉状态下使用的损伤模型——隐静脉出血法,对较低剂量的促凝血药物较为敏感30,但需要实验人员高度介入,因为血凝块必须频繁破坏(相比之下,本文所述模型仅需3次)。

建立统一的标准化方案以测试新型促凝血化合物,将极大 有助于不同实验室之间的数据比较31,32,33。在TVT模型中,目前对于研究终点(如失血量7,26、出血时间9,34和生存率35,36)尚未达成共识,且不同研究之间的实验时长也存在差异13

我们的主要目标是描述并表征一种具有高重复性的优化模型,该模型可用于按需研究以及预防性治疗,对药物干预的敏感性与生存模型相当,但不以死亡或濒死作为实验终点。为了减轻动物的疼痛和痛苦,动物在出血过程中应处于无意识状态,同时需要采用更为伦理的实验终点37

尾部截断模型通常采用两种方式之一进行,一种是截除尾尖部分,例如截除1-5 mm18,19,20,21,23,24;另一种更严重的模型则是在尾部直径约为1-3 mm处进行横断8,22,25。这种操作会导致动静脉联合出血,因为通常会切断侧静脉、背侧静脉和腹侧动脉。一般而言,截除范围越大,对促凝血化合物的敏感性越低。此外,由于尾尖被截除,动静脉损伤处暴露而无对合组织;因此,至少在理论上,该模型与最常见的血友病出血情况有所不同。

顾名思义,在尾静脉横断模型(如本文所述)中,仅静脉受到损伤,因此仅发生静脉性出血 。由于血管并未完全切断,预计损伤程度较截肢模型更小,且保留了切口周围的组织,血凝块可附着于该组织。此外,静脉内的血压低于动脉。这些因素使得该模型相较于截肢模型具有更高的敏感性,因而能够使用临床相关剂量的替代疗法实现出血的正常化,例如在A型血友病中使用重组FVIII(rFVIII),这对于评估促凝治疗效应的强度和持续时间具有重要意义26,38,39

方案

本方案中所述所有操作均已获得诺和诺德公司动物福利委员会以及丹麦动物实验监察局、丹麦食品农业与渔业部的批准。优化后的40分钟方法在设计中包含了麻醉和给药时间(图1)。本操作需使用10至16周龄的雄性和雌性血友病小鼠。

1. 研究前的准备工作

  1. 配制正确浓度的给药溶液。
  2. 启动水浴并加热至 37 °C。用生理盐水(0.9% NaCl)填充用于血液采集的 15 mL 离心管。
  3. 在实验开始前至少 15 分钟,将装有生理盐水的 15 mL 管放入已预热的底板孔中。
  4. 对小鼠进行标识并记录其体重。尽量避免不必要的抓取,以免造成应激反应而影响实验结果。
  5. 在通风橱内提前准备好工作台,确保所有物品触手可及:纸巾、尾部固定器、纱布、注射器、手术刀、秒表和血流记录纸。
  6. 将尾部标记和切割块放置在加热板上——低温的模块会导致静脉收缩,从而影响出血。

2. 麻醉

  1. 在通风橱内进行异氟烷麻醉操作。
  2. 将气体挥发器初始设置为5%异氟烷,载气为30% O2/70% N2O,气体流速1 L/min,在麻醉舱内通气。确保麻醉舱充分充气(约5分钟,具体时间取决于舱体体积和气体流速)。 确保动物快速进入麻醉状态(少于一分钟)。
  3. 将小鼠置于麻醉舱内直至意识丧失。
    注意:若麻醉舱已充分充气,此过程应在一分钟或更短时间内完成。
  4. 通过检测足底反射(用力夹捏脚趾)无疼痛反应,确认麻醉程度适当。
  5. 将小鼠放置于加热板上,确保其鼻部位于鼻罩内。
  6. 将麻醉维持在2%异氟烷(载气为30% O2/70% N2O),并在小鼠上方覆盖塑料罩以减少热量散失。使用合适的眼部软膏涂抹眼睛,防止麻醉期间角膜干燥。
  7. 使用尾部标记块在尾部直径为2.5 mm处做标记。切勿强行将尾部插入标记块的狭缝中——尾部应紧密贴合(补充图1)。
  8. 将尾部浸入生理盐水管中至少5分钟,以确保尾静脉充分扩张,便于静脉(i.v.)给药。

3. 测试溶液的给药

  1. 将一只小鼠放入尾部固定器中,使其鼻部处于麻醉面罩内。
  2. 给予动物目标化合物(本例中为 rFVIII),并立即启动秒表(t = 0)。
  3. 将小鼠放回加热板上,尾巴置于生理盐水管中。对其他小鼠重复此操作。

4. 进行尾静脉切断

  1. 给药后精确5分钟进行尾静脉横切。将尾巴放入切割槽中,并旋转90°以暴露静脉。补充图2).
  2. 在与测试溶液施加位置相对的一侧/静脉进行切割 给药。
  3. 用11号手术刀片穿过固定尾部的切块装置的切口,以造成出血。重置秒表,并立即将尾部放回生理盐水中。

5. 观察时间与挑战

  1. 观察出血情况,并在40分钟内记录出血的开始和停止时间;将记录标注在血流记录纸上。
    注意:由于主观因素,这种对出血的视觉评估可能会有轻微差异。
  2. 造口后3分钟内必须停止主要出血。若未满足此条件,则剔除该小鼠,实施安乐死并更换(主要出血无法停止可能提示损伤过重或初级止血功能缺陷,如vWF基因敲除小鼠)。
  3. 若在损伤后10分钟、20分钟和30分钟均无出血,按步骤4-5所述对尾部切口进行挑战刺激。
  4. 使用另一支预置于水浴中的温盐水浸湿的纱布拭子。将尾巴从盐水中取出,用湿润纱布朝远端方向轻轻擦拭切口处两次。
  5. 每次刺激后,立即将尾巴重新浸入盛有盐水的试管中。
  6. 在t=40 min时,将尾巴从盐水试管中取出,停止血液收集。

6. 血液采样

  1. 在 t = 40 分钟后,从眶上静脉采集血样。

7. 安乐死

  1. 在小鼠仍处于完全麻醉状态下,通过颈椎脱位法实施安乐死。

8. 样品处理

  1. 将含有生理盐水的15 mL采血管在室温下以4000 × g 离心5分钟。
  2. 弃去15 mL管中的上清液,将沉淀物重悬于2–14 mL红细胞(RBC)裂解液中,然后稀释至呈现浅咖啡色。
  3. 记录总体积(血液体积 + 根据管壁刻度加入的红细胞(RBC)裂解液体积)。
  4. 取2 mL稀释液转移至血红蛋白管中,并冷藏至进行血红蛋白分析。
  5. 通过测定生理盐水中血红蛋白浓度来确定失血量。使用酶标仪在550 nm处测定吸光度(材料表)。
  6. 利用人血红蛋白(材料表)制备的标准曲线将吸光度转换为血红蛋白的nmol数,并根据红细胞裂解液的稀释倍数进行校正。

9. 统计分析

  1. 使用适当的软件分析数据。本研究中使用了 GraphPad Prism 软件。在一系列研究中,以下统计方法表现良好。
    注意:对于血液丢失量、出血时间、暴露情况、血小板计数和红细胞压积的分析,采用 Brown-Forsythe 和 Welch 单因素方差分析(ANOVA)检验(因为数据为连续性数据,但残差方差不齐),并使用 Dunnett 检验对多重比较进行校正。采用 Pearson 检验分析出血时间、血液丢失量与剂量之间的相关性。为确定 ED50 值,将四参数逆对数(剂量)反应方程拟合至出血时间和血液丢失数据。为分析性别效应,采用双因素方差分析(two-way ANOVA),并使用 Bonferroni 校正法对多重比较进行调整。显著性水平定义为 P < 0.05。数据以均值 ± 标准误(SEM)表示。

结果

为评估优化模型的适用性,在F8-KO(C57BL遗传背景)小鼠中开展了一项研究,给予 commercially available 重组因子VIII替代疗法(rFVIII);测试了四种不同剂量:1 IU/kg、5 IU/kg、10 IU/kg 和 20 IU/kg。此外,我们还在F8-KO小鼠中测试了相应的载体(阴性)对照,并以C57BL小鼠作为阳性对照组的野生型(WT)组,以评估该模型中的反应范围。

按照优化后的实验方案,与对照组相比,rFVIII各治疗组的失血量显著减少。此外,5 IU/kg、10 IU/kg和20 IU/kg治疗组的出血时间也较对照组缩短(图2)。在野生型(WT)小鼠中,总失血量范围为201.8–841.9 nmol Hgb(95% CI),而对照组小鼠的失血量范围为5335–7148 nmol Hgb(95% CI)。根据约1000 nmol Hgb相当于125 µL全血的换算关系,对照组小鼠的平均出血量为780.25 µL,而20 IU/kg组为89.95 µL。因此,20 IU/kg剂量可使出血完全恢复正常水平,而10 ...

讨论

与尾静脉切开(TVT)存活法相比,这种优化的尾静脉横断(TVT)方法具有多项优势。实验动物在整个研究过程中均处于完全麻醉状态,这不仅使小鼠操作更为简便,也提升了动物福利。此外,与TVT存活模型不同,该方法无需过夜观察,且可在40分钟内精确测量失血量并记录确切的出血时间。在清醒动物中长时间出血可能导致失血性死亡,引发动物疼痛与痛苦,甚至造成应激反应,进而可能增加实验数据的变异性14。失血量和出血时间已被成功地表征并验证为有效的实验终点指标,可替代以死亡或濒死时间为终点的传统方法。由于采用切割块导板进行TVT操作,实际的尾部损伤部位明确且标准化,确保切口具有良好的可重复性,降低了手术难度和技术差异。因此,该模型具有更高的稳健性,可减少所需实验动物数量。此外,已有数据表明,雄性和雌性小鼠均可用于该模型,符合3R原则,从而减少所需繁育的动物总数。结合这些观察结果还发现,在高出血组中,雌性小鼠出血量略少且个体间变异略大,但在低出血组中无此差异。较低的失血量可能与雌性小鼠体型较小、相对于同龄雄性血液总量较少有关。
