Hematopoietic Cell Transplantation

Hematopoietic cell transplantation is a medical procedure that replaces or restores blood-forming stem cells to reestablish healthy blood and immune-cell production. Before transplantation, conditioning therapy with chemotherapy, radiation, or both suppresses the recipient’s marrow and immune system; infused hematopoietic stem and progenitor cells then migrate to the bone marrow, engraft, and generate new blood cells. Cells may come from the patient (autologous) or a donor (allogeneic), with donor immune cells sometimes producing a graft-versus-leukemia effect. The procedure treats blood cancers, bone marrow failure, inherited blood disorders, and selected immune deficiencies, while requiring careful management of infection, graft-versus-host disease, and relapse.

This protocol details the derivation of transplantable hematopoietic stem cells from mouse embryonic stem cells (ESC) and their subsequent injection into lethally irradiated recipient mice. Briefly, ESC are differentiated as embryoid bodies, which are then infected with retroviral HoxB4 and co-cultured with OP9 stromal cells and hematopoietic cytokines.

Quantification of donor-derived cells is required to monitor engraftment after stem cell transplantation in patients with hemoglobinopathies. A combination of flow cytometry-based cell sorting, colony formation assay, and subsequent analysis of short tandem repeats may be used to assess the proliferation and differentiation of progenitors in the erythroid compartment.
