Pulmonary Vascular Impedance

Pulmonary vascular impedance is the frequency-dependent opposition that the pulmonary circulation presents to pulsatile blood flow, providing a more complete measure of right ventricular afterload than vascular resistance alone. It reflects the combined effects of pulmonary vascular resistance, arterial compliance, characteristic vessel properties, and pressure-wave reflection as blood moves from the right ventricle through the pulmonary arteries. Researchers assess impedance from relationships between pulmonary artery pressure and flow across different frequencies. This analysis helps characterize pulmonary hypertension, evaluate right ventricular–pulmonary artery coupling, and distinguish changes in vascular stiffness or wave transmission that may influence diagnosis, treatment assessment, and cardiovascular research.

Chronic Thromboembolic Pulmonary Hypertension (CTEPH) and Right Ventricular (RV) dysfunction were induced in piglets by progressive obstruction of the pulmonary arteries. Consequences were remarkably similar to those observed in CTEPH patients. This animal model would be a very useful tool for pathophysiology and therapeutic experiments on CTEPH and RV failure.

The endothelial glycocalyx/endothelial surface layer is ideally studied using intravital microscopy. Intravital microscopy is technically challenging in a moving organ such as the lung. We demonstrate how simultaneous brightfield and fluorescent microscopy may be used to estimate endothelial surface layer thickness in a freely-moving in vivo mouse lung.
