Surgical Site Irrigation

Surgical site irrigation is the controlled rinsing of an operative field with a sterile solution to remove blood, tissue debris, and potential contaminants before wound closure. In neurosurgery, irrigation typically uses gravity, syringes, or low-pressure systems to deliver fluid across the exposed site, while suction removes the resulting mixture; solution choice, volume, temperature, and pressure must be managed to avoid tissue injury and preserve the surgical field. This practice supports visualization during procedures involving the brain, spinal cord, or peripheral nerves and may help reduce residual contaminants, although its effects depend on technique and clinical context. Standardized irrigation methods are therefore relevant to infection prevention and surgical safety.

Design Example: Design of an Irrigation Channel

Trapezoidal channels are widely used in irrigation systems due to their cost-effectiveness and efficiency in conveying water. Trapezoidal channels feature a flat bottom and sloping sides, making them stable and easier to construct compared to other shapes. The bottom width and side slope ratio are determined based on the required flow capacity and site conditions. The side slope is kept gentle for unlined channels to prevent soil erosion.Hydraulic parameters in channel design include the flow...

Source: Rennert, R. C., et al. Comprehensive Endovascular and Open Surgical Management of Cerebral Arteriovenous Malformations. J. Vis. Exp. (2017).This video demonstrates the open surgical resection of a cerebral arteriovenous malformation (AVM) in a human brain. The procedure involves creating a bone flap, accessing the subarachnoid space, identifying and occluding feeding arteries and draining veins, and carefully removing the AVM. Intraoperative angiography confirms complete resection,...

There are many available options for management of the patient with trigeminal neuralgia. Microvascular decompression, while the most invasive of all options, is also the most effective at achieving long term remission of symptoms. Video instruction on how to maximize efficacy and minimize complications with this procedure is described.

Surgery is the gold standard for accessible arteriovenious malformations (AVMs), and pre-operative embolization can simplify this procedure. We describe our approach for staged endovascular embolization and open resection of AVMs, and provide a representative clinical example highlighting the advantages of a comprehensively trained neurovascular surgeon leading a multi-disciplinary clinical team.
