Endovascular Embolization

Endovascular embolization is a minimally invasive technique that intentionally blocks blood flow within a targeted vessel or vascular lesion, often to treat conditions affecting the brain and spinal cord. During the procedure, clinicians guide a catheter through the vasculature and release embolic materials, such as coils, particles, or liquid agents, which lodge in the target and promote occlusion or clot formation. In neuroscience, this approach can reduce the risk of rupture from cerebral aneurysms, limit blood flow to arteriovenous malformations, and control bleeding or tumor vascularity. Its precision can lessen surgical trauma while providing an important option for managing complex cerebrovascular disease.

Surgery is the gold standard for accessible arteriovenious malformations (AVMs), and pre-operative embolization can simplify this procedure. We describe our approach for staged endovascular embolization and open resection of AVMs, and provide a representative clinical example highlighting the advantages of a comprehensively trained neurovascular surgeon leading a multi-disciplinary clinical team.

A New Murine Model of Endovascular Aortic Aneurysm Repair

Histological and biochemical modifications after aneurysm endograft exclusion are still unclear. We describe a new model of endograft implantation on a murine aneurysm. Through thrombus analysis with and without persistant circulating blood stream interface, and new endovascular biomaterials evaluation, this model will help to better understand endovascular aneurysm exclusion pathobiology.

Simulation of complex, high-risk procedures is critical to the education of medical trainees. A protocol for simulator-based endovascular neurosurgery training in a controlled academic environment is described. The protocol includes stepwise guidelines for trainees of varying levels, with a discussion of the advantages and limitations of this model.

Combination of High Ligation and Intraoperative Embolization Using Polidocanol for Treatment of Varicoceles

2023

To reduce the postoperative recurrence rate of varicocelectomy, we combined high ligation of varicocele with intraoperative embolization. We injected polidocanol from the spermatic vein during surgery to embolize the branches of the spermatic vein and collateral veins. This is an alternative surgical method for the treatment of varicocele.
