End Organ Perfusion

End-organ perfusion is the delivery of oxygenated blood to vital tissues, a core determinant of cellular function and organ survival in medicine. It depends on cardiac output, arterial pressure, vascular resistance, and microcirculatory flow, which together regulate oxygen and nutrient transport and remove metabolic waste; autoregulatory mechanisms adjust blood flow to local tissue needs. Clinicians assess perfusion during conditions such as shock, sepsis, trauma, and cardiac failure using findings that include mental status, skin temperature, capillary refill, urine output, blood pressure, and serum lactate. Recognizing impaired perfusion supports timely resuscitation, guides treatment, and helps prevent irreversible organ injury.

Donor heart preservation is a major determinant of success in heart transplantation. Technological advances have led to optimizing the conditions of preservation, along with a more accurate assessment of the donor heart before transplantation. We herein describe two innovative technologies currently applied for cardiac preservation: uniform cooling preservation and normothermic ex-situ heart perfusion.

Marginal grafts, such as fatty livers, grafts from older donors, or livers retrieved after cardiac death (DCD) tolerate conventional, cold static storage only poorly. We developed a novel model of subnormothermic ex vivo liver perfusion for preservation, assessment, and repair of marginal liver grafts prior to transplantation.

Whole Animal Perfusion Fixation for Rodents

Here we describe a low-cost, rapid, controlled and uniform fixation procedure using 4% paraformaldehyde perfused via the vascular system: through the heart of the rat to obtain the best possible preservation of the brain.
