Trans-psoas Approach

The trans-psoas approach is a minimally invasive surgical route to the lumbar spine that passes laterally through the psoas muscle, providing access while limiting disruption of posterior spinal muscles and ligaments. Surgeons position the patient laterally, use imaging and neural monitoring to identify a safe corridor, gently dilate the psoas, and access the intervertebral disc for procedures such as lumbar interbody fusion. This technique can restore disc height, improve spinal alignment, and indirectly enlarge narrowed neural foramina in selected patients. Understanding the approach is important because its benefits must be balanced against potential psoas injury, nerve irritation, sensory changes, and hip-flexion weakness.
