Ligamentum Flavum Resection

Ligamentum flavum resection is a spinal decompression procedure that removes part or all of the ligamentum flavum, a flexible ligament lining the posterior spinal canal, when thickening contributes to nerve compression. During surgery, the surgeon carefully excises hypertrophied ligamentous tissue, often with other decompressive steps, to enlarge the spinal canal or neural foramen and relieve pressure on the spinal cord or nerve roots. The technique is used in conditions such as lumbar or cervical spinal stenosis and may reduce pain, numbness, weakness, or impaired mobility caused by compression. Its success depends on addressing the underlying anatomy while preserving spinal stability and minimizing tissue injury.

We present a methodology for using an ultrasonic osteotome combined with a conventional osteotome for laminectomy in thoracic ossification of the ligamentum flavum. This is a relatively safe and easy-to-learn method that avoids the perioperative complications associated with the conventional method.
