Cortical Hyperexcitability Assessment

Cortical hyperexcitability assessment evaluates abnormally increased responsiveness in the brain’s cerebral cortex, where neurons may fire too readily or respond excessively to stimulation. Clinicians and researchers estimate this state by applying controlled stimulation and measuring cortical responses with neurophysiological methods such as transcranial magnetic stimulation or electroencephalography, including changes in evoked activity, inhibition, and facilitation. These assessments help characterize altered motor and sensory circuit function in conditions linked to seizures, migraine, neurodegeneration, and other neurological disorders, while supporting disease monitoring, treatment evaluation, and research into the balance between cortical excitation and inhibition.

A high resolution ex vivo 7T MR imaging protocol is presented, to perform MR-guided histopathological validation of microvascular pathology in post-mortem human brain tissue. Furthermore, guidelines are provided for the assessment of cortical microinfarcts on in vivo 7T as well as 3T MR images.

Objective and easy measurement of sensory processing is extremely difficult in nonverbal or vulnerable pediatric patients. We developed a new methodology to quantitatively assess infants and children's cortical processing of light touch, speech sounds, and the multisensory processing of the 2 stimuli, without requiring active subject participation or causing discomfort in vulnerable patients.
