Root Cause Analysis

Root Cause Analysis is a systematic method for identifying the underlying factors that contribute to an unwanted event, rather than focusing only on its immediate cause. In clinical settings, teams reconstruct the event timeline, examine causal pathways and contributing conditions, and use tools such as the “5 Whys” or fishbone diagrams to distinguish individual actions from process, communication, equipment, and organizational factors. The findings support targeted corrective and preventive actions, including workflow redesign, improved documentation, staff training, and safety controls. By addressing system weaknesses, Root Cause Analysis strengthens patient safety, reduces recurrence, and promotes learning from adverse events and near misses.

Promoter expression analyses are crucial to improving the understanding of gene regulation and the spatiotemporal expression of target genes. Herein we present a protocol to identify, isolate, and clone a plant promoter. Further, we describe the characterization of the nodule-specific promoter in the common bean hairy roots.

Full-root aortic valve replacement by stentless aortic xenograft is a viable option in patients with small aortic roots. We describe, a technique for the full-root implantation of stentless aortic xenografts, with emphasis on the management of the proximal suture line and coronary anastomoses, and discuss its limitations and alternative options.
