Donor Heart Assessment

Donor heart assessment is the clinical evaluation of a potential donor heart to determine whether it is suitable for transplantation and likely to function safely in a recipient. Clinicians integrate donor history, physical findings, electrocardiography, echocardiography, hemodynamic data, laboratory tests, and, when indicated, coronary angiography to assess cardiac structure, contractility, perfusion, injury, and transmissible risks. This assessment supports donor-recipient matching and organ allocation while identifying reversible abnormalities or contraindications, helping transplant teams estimate the risk of primary graft dysfunction and make timely decisions that expand safe access to heart transplantation.

This paper describes the protocol for use in static cold storage of rodent donor hearts; however, the protocol can also be used for donor hearts obtained after donation after circulatory death and brain death.
