Granuloma Caseum

Granuloma caseum, also called a caseating granuloma, is an organized inflammatory lesion in which immune cells surround a central area of cheese-like necrosis. It matters because this pattern can signal a persistent infectious process, especially in medical pathology. During formation, macrophages transform into epithelioid cells and multinucleated giant cells, while lymphocytes help maintain the cellular wall around material that the immune system cannot readily eliminate. On biopsy, recognizing granuloma caseum helps clinicians interpret tissue architecture and prioritize microbiological testing, including evaluation for tuberculosis or selected fungal infections, although similar necrotic patterns may occur in other conditions.

Here we describe a rapid equilibrium dialysis (RED) method to measure drug binding to caseum from pulmonary tuberculosis lesions and cavities. The protocol is also used with a foamy macrophage-derived matrix that is an effective surrogate to caseum.

Mycobacterium leprae, the causative agent of leprosy, does not grow in vitro. We describe an easy to follow protocol to prepare a bacillary suspension to ensure the maintenance of large quantities of M. leprae for a variety of applications. Protocols for propagation by mouse footpad inoculation, evaluation of viability, freezing and thawing bacillary stock are described in detail.

Schistosoma mansoni eggs are potent stimulators of the T helper type 2 (Th2) immune response, characteristic of parasite infection, asthma and allergic inflammation. This protocol utilizes S. mansoni egg injection to generate a CD4 Th2 cytokine-induced inflammatory response in the lung, characterized by lung granuloma formation around the egg, eosinophilia and macrophage alternative activation.
