Therapeutic Efficacy Monitoring

Therapeutic efficacy monitoring is the systematic assessment of how well a treatment produces its intended biological or clinical effect, helping determine whether therapy should continue, change, or stop. It combines measurements such as target engagement, biomarker levels, physiological responses, and patient outcomes with predefined treatment goals, often comparing results over time to distinguish meaningful benefit from inadequate response. In bioengineering, engineered biosensors, diagnostic platforms, imaging systems, and computational models can provide quantitative, time-resolved data for this assessment. These tools support personalized treatment, improve dose and intervention decisions, and accelerate the development of safer, more effective therapies.

Bioimaging methods used to assess cell biodistribution of nanoparticles are applicable for therapeutic and diagnostic monitoring of nanoformulated compounds. The methods described herein are sensitive and specific when assessed by histological coregistration. The methodologies provide a translational pathway from rodent to human applications.

We describe methods for longitudinal monitoring of the efficacy of therapeutics for the treatment of colonic pathologies in mice using a rigid endoscope. This protocol can be readily used for the characterization of the therapeutic response of an individual tumor in live mice and also for monitoring potential disease relapse.

We demonstrate how changes in the intracellular free calcium concentration and synaptic efficacy can be simultaneously monitored in a ganglion preparation of Aplysia. We image intracellular calcium using a fluorescent dye, Calcium Orange, and induce and monitor synaptic transmission with sharp (intracellular) electrodes.
