Pleural Friction Rub

Pleural friction rub is an abnormal, superficial scratching or grating sound heard with a stethoscope when inflamed pleural surfaces move against one another, making it an important clinical sign of pleuritis. Normally smooth, the visceral and parietal pleura glide quietly; inflammation roughens these surfaces, producing sound during breathing, often near the area of pleuritic pain and during both inspiration and expiration. Recognizing this finding supports evaluation of pleuritic chest pain and conditions such as pneumonia, pulmonary embolism, or autoimmune pleural disease, although clinicians interpret it alongside the patient’s history, examination, and imaging.
