Septal Myectomy

Septal myectomy is an open-heart surgical procedure that removes part of an abnormally thickened interventricular septum, most commonly to treat obstructive hypertrophic cardiomyopathy when excess muscle limits blood leaving the left ventricle. During the operation, a surgeon excises targeted septal tissue to enlarge the left ventricular outflow tract, reducing pressure gradients and often relieving obstruction caused by systolic anterior motion of the mitral valve. In clinical care, the procedure can improve exertional symptoms and cardiac function in appropriately selected patients, making it an important treatment for severe, medication-refractory obstruction and a model for managing structural causes of impaired blood flow.

Endoscopic septoplasty is a time-honored surgical procedure with multiple variations. This paper focuses on a step-by-step surgical approach to perform a modified septoplasty procedure known as limited two-line resection. This surgical technique can be applied to correct a deviated nasal septum in the absence of an external nasal deformity.

Fetal and perinatal death is a common feature when studying genetic alterations affecting cardiac development. High-frequency ultrasound imaging has improved 2-D resolution and can provide excellent information on early cardiac development and is an ideal method to detect the impact on cardiac structure and function prior to death.

Here, we present a protocol for recording rhythmic neuronal network theta and gamma oscillations from an isolated whole hippocampal preparation. We describe the experimental steps from extraction of the hippocampus to details of field, unitary and whole-cell patch clamp recordings as well as optogenetic pacing of the theta rhythm.
