Composite Tissue Allotransplantation

Composite tissue allotransplantation is the transplantation of multiple tissue types, such as skin, muscle, bone, nerves, and blood vessels, from a donor to restore complex anatomy and function in a recipient. Because the graft contains genetically different cells, the recipient’s immune system recognizes donor antigens as foreign and can initiate cellular and antibody-mediated rejection, requiring immunosuppressive therapy and careful monitoring for infection. Used in selected cases of severe limb, face, and other tissue loss, this approach can improve physical function and quality of life while illustrating the close relationship between transplantation immunology, immune tolerance, and infectious risk. Ongoing research seeks safer strategies that preserve graft survival while reducing long-term immunosuppression.

Currently any kind of vascularized composite allotransplantation depends on long-term-immunosuppression, difficult to support for non-life-critical indications. We present a new porcine tibial VCA model that can be used to study bone VCA and demonstrate the use of surgical angiogenesis to maintain bone viability without the need of long-term immune-modulation.
