Flap Perfusion Assessment

Flap perfusion assessment is the evaluation of blood flow in tissue transferred during reconstructive surgery to determine whether the flap remains viable. It combines clinical findings, such as skin color, temperature, capillary refill, and bleeding, with tools including handheld Doppler ultrasound or indocyanine green fluorescence imaging to assess arterial inflow and venous drainage. Early detection of inadequate perfusion helps clinicians identify vascular compromise before irreversible tissue injury develops, enabling interventions such as flap repositioning or vascular revision. In medicine, these assessments support safer free-tissue transfer, improve postoperative monitoring, and reduce complications including partial or complete flap loss.

Marginal grafts, such as fatty livers, grafts from older donors, or livers retrieved after cardiac death (DCD) tolerate conventional, cold static storage only poorly.
