Gastric Stromal Tumors

Gastric stromal tumors are mesenchymal neoplasms that arise in the stomach, most commonly as gastrointestinal stromal tumors (GISTs), and they matter because their behavior ranges from indolent growth to aggressive metastatic disease. Many GISTs develop through activating mutations in KIT or PDGFRA, disrupting signaling pathways that regulate the survival and proliferation of interstitial cells of Cajal, the stomach’s pacemaker cells. Diagnosis combines endoscopy, imaging, and histopathology, often with immunohistochemical markers such as KIT and DOG1. Treatment may include surgical resection for localized tumors and targeted therapy, particularly imatinib, for tumors with sensitive mutations or advanced disease.

In early gastric cancer, the aim of surgery is to precisely remove the distal stomach including the primary tumor. To do this, accurate localization of the tumor is crucial, especially in totally laparoscopic surgery. This protocol describes a procedure for intraoperative gastroscopy in totally laparoscopic subtotal gastrectomy.

Laparoscopy-endoscopy cooperative surgery is appropriate for the treatment of gastric gastrointestinal stromal tumors less than 5 cm in size. It can achieve the respective advantages of endoscopic and laparoscopic surgeries while avoiding their drawbacks.
