Ischemic Limb Reperfusion

Ischemic limb reperfusion is the restoration of blood flow to a limb after a period of reduced or blocked circulation, a process that can save viable tissue while also causing secondary injury. During ischemia, oxygen and nutrient deprivation disrupt cellular energy production; when circulation returns, renewed oxygen delivery can generate reactive oxygen species, calcium imbalance, endothelial dysfunction, and inflammatory responses that impair the microcirculation. In medicine, understanding this ischemia-reperfusion injury helps guide treatment of acute limb ischemia, vascular trauma, and surgical revascularization. It also supports strategies to preserve muscle and nerve function, reduce tissue damage, and improve outcomes after restoring perfusion.

Remote ischemic preconditioning (RIP) is a method of conditioning tissues against damaging stress. We have established a method of remote ischemia at the hind limb, by inflating a sphygmomanometer cuff for 5-10 min. The neuroprotective capabilities of RIP have been demonstrated in a model of retinal degeneration in rodents.
