Peritoneal Pouch

A peritoneal pouch is a pocket-like recess or fold formed by reflections of the peritoneum, the membrane that lines the abdominal cavity and covers many abdominal organs. These spaces arise where parietal and visceral peritoneum change direction, creating potential compartments that can accommodate organ movement but may also collect fluid, blood, or inflammatory material. Understanding peritoneal pouches helps clinicians interpret cross-sectional imaging, predict the spread or localization of infection and internal bleeding, and plan abdominal and pelvic procedures. Their anatomy is particularly relevant when assessing ascites, abscesses, or other conditions involving the peritoneal cavity.

After excising a segment of the right CCA, the pouch is treated with elastase, then sutured to the left CCA to restore blood flow. An optimized technique for the microsurgical creation of arterial bifurcation aneurysms mimicking bifurcation human cerebral aneurysms is described. A venous pouch is sutured into an artificially created true bifurcation of both common carotid arteries. Facilitated microsurgical techniques and aggressive postoperative anticoagulation and analgesia lead to minimized morbidity rates and high aneurysm patency rates.
