Upper Gastrointestinal Surgery

Upper gastrointestinal surgery is the surgical treatment of disorders affecting the esophagus, stomach, and duodenum, including cancer, obstruction, bleeding, and severe reflux. Procedures may use open or minimally invasive access to remove diseased tissue, control bleeding, relieve blockage, or reconstruct the digestive tract; surgeons often restore continuity through anastomosis while preserving nutrition and organ function. These operations support cancer treatment, manage complex emergencies, and address conditions that do not respond to medical therapy. Advances in laparoscopy, endoscopy, and perioperative care aim to reduce tissue trauma, shorten recovery, and improve long-term outcomes.

Laparoscopy-endoscopy cooperative surgery is appropriate for the treatment of gastric gastrointestinal stromal tumors less than 5 cm in size. It can achieve the respective advantages of endoscopic and laparoscopic surgeries while avoiding their drawbacks.

This protocol describes the use of excised intestinal tissue preparations or "intestinal sacs" as an ex vivo model of intestinal barrier function. This model may be used to assess integrity of both the epithelial barrier and the mucous gel layer at specific intestinal sites in animal models of digestive disease.

Here, a protocol to isolate and establish primary fibroblast/myofibroblast (MF) cultures from frozen gastric, small intestinal, and colonic tissue-yielding cells with a MF phenotype-is presented. These cells express CD90, α-SMA and vimentin. MFs can be used for a variety of functional assays including enzymatic activity and cytokine production.

Robotic Duodenal Sleeve Resection for Gastrointestinal Stromal Tumor with Rare Exon 8 KIT Mutation Following Neoadjuvant Imatinib

This video case report demonstrates the unique management of a duodenal gastrointestinal stromal tumor with a rare, understudied, exon 8 KIT mutation. Neoadjuvant therapy resulted in a 30% reduction in tumor size, making the patient a candidate for a robotic-assisted duodenal sleeve resection instead of the more morbid pancreaticoduodenectomy.
