Summary
内镜超声引导下胆道引流 (EUS-BD) 是恶性胆道梗阻的胆道减压的替代方法。在这里,我们描述了EUS引导下肝胃造瘘术(EUS-HGS)在不可切除的恶性肺门胆道梗阻病例中的技术。
Abstract
不可切除的恶性胆道梗阻患者通常需要胆道引流来减压胆道系统。内镜逆行胰胆管造影术 (ERCP) 是尽可能的主要胆道引流方法。经皮经肝胆道引流 (PTBD) 在 ERCP 失败时用作挽救方法。内镜超声引导下胆道引流 (EUS-BD) 提供了一种可行的替代胆道引流方法,其中一种方法是 EUS 引导的肝胃造瘘术 (EUS-HGS)。在这里,我们描述了EUS-HGS技术在不可切除的恶性肺门胆道梗阻的情况下实现胆道引流。

Introduction
恶性胆道梗阻患者在就诊时通常不可切除和进展1,2。因此,在治疗这些病例时,通常需要姑息性内镜胆道减压术3,4,5。根据目前的建议,内镜逆行胰胆管造影术(ERCP)是胆道引流的主要方法,只要有可能,如果失败或有禁忌,经皮经肝胆道引流(PTBD)被用作挽救方法6,7,8。然而,PTBD存在某些并发症,包括败血症,胆管炎,出血,电解质丢失,泄漏,伤口感染,局部不适;据报道,这些并发症高达53.2%9。内镜超声引导下胆道引流 (EUS-BD) 的出现为解决这一差距提供了一种可行的替代胆道引流方法。EUS-BD的主要技术涉及使用内森成像来引导 通过 胃肠道进入胆道系统,以提供阻塞性胆道系统的治疗性减压。
EUS-BD 于 2001 年首次实施,从那时起,这种胆道引流方法多年来不断发展10。EUS-BD的方法通常是EUS引导的胆总管十二指肠造口术(EUS-CDS),EUS引导的肝胃造瘘术(EUS-HGS),EUS引导的顺行支架置入术(EUS-AS)和EUS引导的会合(EUS-RV)11,12。迄今为止, EUS - BD 的适应证包括 ERCP 失败的患者,十二指肠梗阻无法进入的患者以及手术解剖结构改变的患者 13 , 14 , 15 。
EUS-HGS 涉及将左侧肝内导管经壁引流至胃部。主要优点是它提供内部引流,与PTBD相比,它更具生理性,最重要的是,它提供了更好的患者舒适度12,16。该方法对肺门和远端恶性胆道梗阻均可行。在这里,我们将EUS-HGS的技术描述为EUS-BD在不可切除的恶性肺门肿瘤病例中的方法之一。
1名71岁女性出现无痛性黄疸,体重减轻4 kg,持续2周。经检查发现她患有巩膜黄疸。腹部检查不明显。计算机断层扫描(CT)成像显示4 x 5cm肺门肿瘤,涉及右肝管分叉,肝导管和肝外导管汇合,引起明显的肝内导管扩张伴淋巴结肿大和肝转移(图1A)。对病变进行EUS细针活检(FNB),结果显示腺癌呈CK7和CA19-9阳性,与肺门胆管癌的诊断一致(图1B)。就诊时胆红素水平为212 μmol/L(<15),CA19-9水平为305 U/mL。进行EUS-HGS以缓解胆道梗阻。

Protocol

1. 定位和镇静
- 将患者置于俯卧姿势。使用静脉输注丙泊酚提供中度镇静剂。确保麻醉师滴定输液。
2. 确定合适的目标扩张的左肝内导管
- 推进线性超声内窥镜,确保其通过胃食管连接处。
- 将超声内窥镜的尖端放在胃腔的较小曲率处,然后继续识别扩张的左肝内导管(IHD)。
- 进行透视成像以确定超声内窥镜的位置。如前所述,超声内窥镜的尖端必须放置在胃腔的较小曲率处。始终保持超声镜位置,并在整个过程中使用荧光镜成像进行定期检查以稳定。
- 识别扩张的段 3 IHD。稍微向上倾斜超声内窥镜尖端的角度,以利于针刺的下一步。
注:该过程的后续步骤包括在整个过程中 通过 回声内窥镜的工作通道插入和更换附件。
3. 针头进入目标扩张的左肝内导管
- 执行多普勒超声检查,以确保目标IHD周围没有干预血管。
- 使用19 G针刺穿3段IHD(图2A)。使用预装有7 mL造影剂溶液的10 mL注射器,轻轻抽吸胆汁溶液,以确认胆道通路的成功。
- 之后,进行造影剂注射以遮蔽左侧IHD和胆道系统的其余部分。
4. 导丝操作
- 使用 0.025 英寸导丝在透视引导下导航到左侧 IHD。
- 将导丝穿过右侧肝内导管(图2B)。
注意:理想情况下,导丝应深入胆总管;然而,由于阻塞性肺门肿瘤,在这种情况下是不可能的。
5. 瘘管扩张
- 除使用4 mm胆囊扩张器外,还使用6 Fr电烙术扩张器扩张瘘管(图2C)。
注意:需要瘘管扩张,以方便以后插入胆道支架。 - 执行气球充气约 5 秒。
注意:在执行此操作时,使用超声和透视图像监视所监视的附件的位置,确保导线可见并保持回声内窥镜位置。这样做是为了确保在更换配件期间平稳过渡,这非常重要。
6. 支架插入和部署
- 在透视引导下部署一个10毫米大小的部分覆盖的胆道支架(长度为10厘米)(图3A)。
注意:支架有一个3厘米的未覆盖的远端部分和一个7厘米覆盖的近端部分。未覆盖的部分必须在肝内导管内(图3C)。支架覆盖部分的约3厘米留在胃腔中,以防止支架向内移动(图3B)。 - 确保支架的远端在IHD中打开。将近端部署在超声内窥镜的工作通道内,随后释放到胃中,看到胆汁在支架内流动。

Representative Results
该过程在大约30分钟内完成。手术后没有并发症,患者能够在第二天恢复口服摄入。胆红素水平降至92μmol/L,手术后三天出院。重复进行CT成像,显示支架就位,胆道梗阻消退。术后2周后随访的胆红素水平为30 μmol/L,术后4周时为14 μmol/L(表1)。
2020年6月至2021年6月期间,共有15例不可切除的恶性胆道梗阻患者在我院接受了EUS-HGS治疗。平均年龄为65.5岁(±10.1岁),男性占46.7%。胰腺癌(46.7%)是最常见的诊断,手术解剖结构改变(33.3%)是该手术最常见的指征。临床成功被定义为在术后4周内胆红素水平降低超过50%。技术成功率、临床成功率和手术相关不良事件分别为86.7%、81.8%和6.7%。唯一与手术相关的不良事件是出血,这需要经动脉栓塞。一名患者在 通过 现有金属支架插入双尾纤塑料胆道支架的手术后102天,由于支架再次闭塞而需要再次干预。两名患者因晚期癌症在不到4周的时间内死亡。我们的经验结果如 表2所示。

Discussion
与ERCP和PTBD等现有方法相比,上述病例描述说明了在治疗胆道恶性肿瘤时使用EUS-HGS作为替代胆道引流方法的可能性。在上述步骤中,识别和访问正确的肝内导管,导丝操作和支架部署是确保手术可以成功安全地进行的三个主要关键步骤。
在选择理想的胆道通道肝脏段时,可以靶向第2段和第3节IHD,但第3节通常优于第217节。这是因为在第2部分中,所使用的装置和附件存在穿过食道并进入纵隔的风险,从而导致纵隔炎和纵隔等并发症18。除此之外,食道内支架开口的风险可能导致食管穿孔延迟进入纵隔19。话虽如此,只要超声镜的位置在胃腔内, 通过 第2段进入并不是禁忌的。
肝内导管内导丝操作至正确的方向和位置可以说是该手术中最困难和最关键的方面。导丝的理想位置应在胆总管内或至少在肝门处,以便为更换配件提供足够的长度和稳定性,而不会有导丝从插入部位滑出的风险17。在上述情况下,由于肿瘤近端梗阻,导丝无法通过胆总管,但能够通过右侧肝内导管以提供额外的导线长度,从而提高稳定性。可以使用亲水性 0.035 英寸导丝或 0.025 英寸导丝,其尖端具有高度柔韧性,可提供足够的刚度17,20。细针抽吸(FNA)针内的导丝剪切并不是一个罕见的问题,因此,在操纵它时应格外小心21。导丝插管成功后扩张瘘管的选择包括使用电烙术装置,带或不联合胆道布吉/球囊扩张器,通常使用6 Fr电烙术装置,瘘管扩张不应超过8.5 Fr以避免胆汁渗漏。然而,当使用电烙术扩张器时,出血的频率更高,因此有些人可能更喜欢仅使用布吉/球囊扩张器或特定的锥形尖端胆道扩张器22,23,24。
关于支架的选择,完全覆盖的自膨胀金属支架,部分覆盖的金属支架和塑料支架可用于EUS-HGS25,26。与塑料支架相比,金属支架通常是首选,因为支架通畅性更好,胆汁渗漏发生率更低13。专用的部分覆盖的混合金属支架,例如在这种情况下使用的支架,已越来越多地用于该程序。这些支架由未覆盖的肝内远端部分和覆盖的胃部组成,并带有近端锚定皮瓣,以防止支架向内迁移和胆汁泄漏27,28,29。与范围外部署相比,支架的作用域内通道部署是目前支架部署的首选方法,以防止支架迁移30。
EUS-HGS在现有研究(以回顾性为主)中的技术成功率和临床成功率分别为65%~100%和66%~100%,而不良事件发生率为9.5%~35.6%16、29、31、32、33、34、35、36、37、38.出血是最常见的不良事件,其中包括腹膜炎、胆管炎、肺腹膜、胆汁渗漏和支架迁移或支架再闭塞14,16,36,39,40。这些结果与我们机构的经验是一致的。与现有的胆道引流方法相比,EUS-HGS 在疗效和安全性方面与 PTBD 相当,但再干预较少,住院时间更短41。总体而言,EUS-BD优于PTBD。这反映在最近的一项荟萃分析中,该分析表明,与PTBD42相比,EUS-BD具有更好的临床成功,更低的不良事件和更低的再干预率。EUS-BD与PTBD相比的另一个优点是它提供远离肿瘤病变的内部胆道引流,从而改善支架通畅。此外,大多数患者更喜欢EUS-BD而不是PTBD,主要原因是它提供内部引流而不是外部引流43。在支架重新闭塞的情况下, 通过 现有支架放置的再干预可以通过重新插入另一个金属支架或塑料胆道支架轻松完成38。因此,如果具备专业知识,当 ERCP 失败时,EUS-BD 可以成为一种挽救性胆道引流方法7,13,14。话虽如此,这是关于使用EUS-HGS作为胆道引流的主要方法并取得良好成功的有希望的证据38,44。
虽然EUS-HGS已被证明是一种可行的胆道引流方法,但其在常规临床实践中的使用存在局限性。首先,这是一个复杂的程序,仅限于具有专业知识的中心和在高级内窥镜超声中具有高病例量的中心。此外,在训练和执行此过程时,存在陡峭的学习曲线45。考虑到这一点,建议仅由具有足够培训和经验的内窥镜医生进行13,14。患者的选择也是选择这种胆道引流方法时要考虑的重要因素。EUS-HGS 不应用于不适合内镜检查、预期寿命差或存在凝血功能障碍的患者,也不应在存在腹水的情况下进行。在存在腹水的情况下,可能无法确定成熟的瘘管,这将导致胆汁和肠漏引起的腹膜炎14。如前所述,EUS-BD 的出血风险通常较高,尽管对于凝血功能障碍的特定类型没有明确的建议,但如果血小板水平低于 50 x 109cells/L 和/或国际标准化比值 (INR) 大于 1.546,则不应进行 EUS-BD。
综上所述,EUS-HGS是恶性胆道梗阻中一种可行且安全的胆道引流方法。目前,ERCP 仍然是胆道引流的主要方法,如果失败或有禁忌证,可考虑 EUS-HGS 或 PTBD。与PTBD相比,EUS-HGS可能具有一些优势,因为它提供内部引流,不良事件发生率较低,再干预率较低。话虽如此,所有这些方法都应该相互补充和共存,以实现良好的临床结果。需要进一步的前瞻性研究来比较这些方法及其长期结局。
