Muscles of the Vertebral Column

Back muscles that lie deep to the thoracolumbar fascia are called intrinsic, or true, back muscles. They are grouped into four layers. These layers are the superficial, intermediate, deep, and deepest layers.

The superficial layer mainly includes the splenius muscles. These are the splenius capitis and splenius cervicis. They help move the head and cervical spine by supporting extension, rotation, and lateral bending. The splenius capitis runs from the upper thoracic and lower cervical vertebrae to the skull. The splenius cervicis extends between the thoracic and cervical vertebrae.

The intermediate layer contains the erector spinae muscles. This group includes the iliocostalis, longissimus, and spinalis. These long, column-like muscles extend almost the full length of the spine. They run from the sacrum and iliac crest to the ribs and vertebrae near the base of the skull. Their main job is to support posture and extend the vertebral column. They also help with lateral flexion, and when one side works alone, they can bend the spine to the side.

The deep layer includes the transversospinalis muscles. These are the semispinalis, multifidus, and rotatores. These shorter muscles help stabilize the spine. They also assist with extension and allow controlled rotation and bending of the vertebral column. The deepest layer includes small deep muscles such as the interspinales and intertransversarii, which connect adjacent vertebrae and support stability and local movement. This layer also includes the levatores costarum, which help raise the ribs during deep inhalation.
