Phrenic Nerve Pathway

The phrenic nerve pathway is the anatomical route taken by the nerves that provide the primary motor supply to the diaphragm, making it essential for breathing and clinically important in respiratory disorders. Each phrenic nerve arises mainly from spinal roots C3–C5, descends through the neck and thorax, and reaches the diaphragm, where action potentials trigger muscle contraction and inspiration; it also carries sensory signals from the diaphragm, pericardium, and mediastinal pleura. Understanding this pathway supports the assessment of nerve injury, diaphragmatic paralysis, respiratory weakness, and referred shoulder pain, while informing surgical planning, imaging, and therapeutic nerve stimulation.

Measuring Compound Muscle Action Potentials by Stimulating the Phrenic Nerve in a Rat

This video describes recording compound muscle action potentials (CMAPs) in diaphragm muscle fibers in a rat through phrenic nerve stimulation. A high-intensity electric pulse stimulation of the phrenic nerve generates action potentials in the diaphragm muscle fibers, which are observed by obtaining the response curve.

Compound muscle action potential recording quantitatively assesses functional diaphragm innervation by phrenic motor neurons. Whole-mount diaphragm immunohistochemistry assesses morphological innervation at individual neuromuscular junctions. The goal of this protocol is to demonstrate how these two powerful methodologies can be used in various rodent models of spinal cord disease.
