Nitroglycerin Infusion

Nitroglycerin infusion is an intravenous medicine that rapidly relaxes vascular smooth muscle to reduce the heart’s workload and improve myocardial oxygen balance. After administration, nitroglycerin releases nitric oxide, which activates soluble guanylate cyclase, increases cyclic guanosine monophosphate, and produces predominantly venodilation, with arterial dilation at higher doses; clinicians adjust the infusion according to blood pressure and symptoms. In medicine, it is used for acute angina, perioperative ischemia, hypertensive emergencies, and selected cases of acute heart failure. Continuous monitoring is essential because excessive vasodilation can cause hypotension, reflex tachycardia, or reduced organ perfusion.

This protocol presents the acetylcholine rechallenge after nitroglycerine as an add-on procedure to spasm provocation testing. The purpose of this technique is to unmask co-existing microvascular spasm in patients with epicardial spasm and to assess the protective efficacy of nitroglycerine on a per-patient level to guide medical therapy.
