Pharmacy-led Framework

A Pharmacy-led Framework is a structured model that places pharmacists at the center of coordinated medicine management, linking medication assessment, prescribing support, dispensing, and follow-up to improve how medicines are used. It works through systematic review of a patient’s medicines, identification of therapeutic problems or safety risks, evidence-informed recommendations, monitoring, and communication with prescribers and patients. In medicine, this approach can support adherence, reduce medication-related problems, and strengthen continuity across care settings. Its value lies in integrating clinical pharmacy expertise into routine decision-making while providing a reproducible basis for safer, more effective, and patient-centered therapy.

2023

Source: Jennifer A. Ouellet and Jaideep Talwalkar; Yale School of Medicine To meet the needs of older adults, all health professionals should be well acquainted with the history and physical examination considerations unique to this population. Physical examination plays an important role in the older patient to detect physiologic changes of aging, risk factors, and signs of pathology. While most of the general principles of the standard examination for adults apply to older patients, there are...

Synthesis, activation, and characterization of intentionally designed metal-organic framework materials is challenging, especially when building blocks are incompatible or unwanted polymorphs are thermodynamically favored over desired forms. We describe how applications of solvent-assisted linker exchange, powder X-ray diffraction in capillaries and activation via supercritical CO2 drying, can address some of these challenges.
