Spontaneous Ventilation Maintenance

Spontaneous ventilation maintenance is the preservation of a patient’s own breathing effort to support adequate ventilation and gas exchange without fully replacing respiratory muscle activity. It depends on intact respiratory drive, coordinated contraction of the diaphragm and other respiratory muscles, and a sufficiently open airway, while clinicians assess breathing pattern, oxygenation, carbon dioxide removal, and overall work of breathing. In medicine, maintaining spontaneous ventilation can help guide respiratory support and clinical decision-making during anesthesia, critical care, and recovery. Careful monitoring is essential because fatigue, airway obstruction, or worsening disease may reduce ventilation and require additional support.

A spontaneous process is one that occurs naturally under certain conditions. A nonspontaneous process, on the other hand, will not take place unless it is “driven” by the continual input of energy from an external source. Processes have a natural tendency to occur in one direction under a given set of conditions. Water will naturally flow downhill (spontaneous process), but uphill flow (nonspontaneous process) requires outside intervention such as the use of a pump. Iron exposed to the earth’s...

A Novel Rescue Technique for Difficult Intubation and Difficult Ventilation

2011

We describe a technique to maintain oxygenation and ventilation using an endotracheal tube inserted nasally to the level of the naso-pharynx while sealing the mouth and nares for successful positive pressure ventilation.

Drosophila Maintenance

Drosophila melanogaster, commonly known as fruit flies, are a frequently used model organism for life science research. Although starting a collection of these critters may seem as easy as leaving a banana on your kitchen counter for too long, a productive fly colony in the lab requires careful husbandry and maintenance. This video demonstrates the necessary steps for maintaining a healthy fly stock. The overview begins with the preparation and storage of the yeast and sugar-containing media on...

Mechanical Ventilation II: Invasive Ventilation

Ventilators are essential medical equipment used to aid patients with respiratory difficulties. Their primary function is to assist or replace spontaneous breathing by providing mechanical ventilation. There are two general classes of mechanical ventilators: negative-pressure and positive-pressure ventilators. Negative-Pressure Ventilators Negative-pressure ventilators create a vacuum around the chest or body to draw air into the lungs, simulating breathing. This method does not require an...

Expired CO2 Measurement in Intubated or Spontaneously Breathing Patients from the Emergency Department

This video describes how to perform CO2 measurement in intubated as well as spontaneously breathing patients. The main clinical indications refer to emergency situations: (1) verifying adequate positioning of an endotracheal tube; (2) achieving normocapnia in trauma patients; (3) monitoring ventilation in the case of procedural sedation.
