利用荧光辅助识别技术检测与去除牙色复合树脂

荧光辅助识别技术是一种可行、快速且可靠的区分复合树脂修复体与牙体组织的方法,有助于微创且完整地去除复合树脂修复体及复合树脂粘接的牙外伤夹板。

摘要

检测和去除牙色充填材料是每位牙医面临的主要挑战。荧光辅助识别技术(FIT)是一种无创性工具,可帮助区分复合树脂材料与健康的牙体组织。与传统照明相比,FIT 是一种非常准确、可靠且快速的诊断方法。当使用约 398 ± 5 nm 波长的光照射复合树脂时,其中特定的荧光成分会使复合树脂比牙体组织更亮。任何能发出合适波长荧光激发光的光源均可用于该方法。理想情况下,该技术应在无额外自然光或人工照明的环境中使用。FIT 可用于诊断目的,例如绘制牙科图表,还可用于复合树脂修复体的完整且微创性去除、托槽去粘接以及外伤夹板拆除。通过叠加修复去除前后的扫描图像,并利用合适的软件进行后续计算,可评估去除复合树脂后的体积变化。

引言

与常规照明(例如牙科综合治疗台的照明灯)相比,荧光诱导透射技术(FIT)的应用有助于区分复合树脂材料与健康的牙体组织1,2。当材料吸收光后以比吸收波长更长的波长发射光时,即发生荧光现象。由于这种照明作用,材料的亮度会高于牙齿本身3。复合树脂材料在受到波长为398 ± 5 纳米的光照射时,荧光强度达到最大值3。复合树脂材料中的荧光来源于添加至玻璃填料中的稀土氧化物,而玻璃填料是复合树脂的主要成分之一4,5。添加这些荧光物质的目的是使复合树脂的光学特性与牙体结构相匹配,从而改善其美学性能4,5。由于多数复合树脂材料具有上述荧光特性,因此FIT适用于多种复合树脂材料3。然而,随着复合树脂材料的老化,其荧光强度会逐渐减弱6,7,8,9

由于现代复合树脂材料的光学特性与牙体组织几乎完全匹配,使用常规照明方式区分复合树脂材料与牙体组织具有挑战性10,11。对复合树脂的误判会导致牙科记录不准确、龋病风险评估错误以及治疗计划不当11。此外,流行病学数据也会因此失真12

由于操作简便、美观性能良好以及临床效果优异,复合树脂是直接修复的首选材料13。然而,许多复合树脂修复体因继发龋、折裂或其他原因需要被更换14,15。但在常规照明条件下,去除残留的复合树脂材料可能具有挑战性。即使使用放大设备,并借助触诊探针或对牙齿进行充分干燥,有时仍难以将复合树脂残留物与健康的牙体组织区分开来。在粘接修复体去除过程中若遗留复合树脂残余,会降低后续修复的质量,并可能因边缘着色而影响美观1,16,17,18,19,20,21,22。相反,若将复合树脂误判为牙体组织而导致过度预备,则可能造成不必要的牙体组织丧失1,2

在牙科创伤学中,使用创伤夹板固定受损牙齿在许多情况下是常见且必需的23。通常使用流动性复合树脂材料将创伤夹板固定在牙齿上。在此情况下,若未能完全清除复合树脂材料,可能导致上述功能障碍。由于牙科损伤多发生在前牙区,美学效果的维持以及后续修复体的充分粘接至关重要。因此,本文旨在介绍FIT方法的应用,作为一种检测和清除复合树脂材料的有效且简便的技术。

方案

本研究中使用的牙齿样本属于经当地伦理委员会(EKNZ UBE-15/111)批准的项目。参与者均签署了书面知情同意书,所有数据均已去标识化,以保护患者隐私。

1. 使用FIT检测牙色复合树脂材料

  1. 关闭房间内的自然光和人工光源,使环境变暗。
  2. 佩戴具有紫外线防护功能的透明或黄色镜片安全护目镜。
  3. 使用可诱导荧光的光源照射牙体组织及牙色复合树脂修复体(图1)。
    注意:复合树脂材料的亮度将高于牙体组织(图2)。任何波长为 398 ± 5 nm 的设备均可作为诱导荧光的光源。头灯系统尤为适用,因其光斑可照亮整个口腔,同时便于进行触诊检查。唾液和菌斑等干扰因素不会影响荧光检测法(FIT)的效果,因此无需事先清洁或反复干燥牙齿。

使用 FIT 去除复合树脂粘接的创伤夹板

  1. 为实验评估目的,使用口内扫描仪及相应软件进行术前扫描
    1. 启动口内扫描仪并打开软件(参见材料表)。
    2. 点击添加新患者以注册患者,填写姓氏、名字、出生日期患者编号,然后点击添加病例
    3. 适应症部分选择颌骨扫描印模,然后点击下一步
    4. 调暗房间光线(自然光和人工光源),并干燥操作区域,以方便扫描过程。
    5. 启动口内扫描仪,对操作区域进行数字化表面扫描(图3A)。
  2. 复合树脂材料的可视化
    1. 重复步骤1.1–1.3。
    2. 使用常规方法(例如,高速反角手机配合金刚砂车针及抛光器械)去除复合树脂材料(图4)。
      注:使用专为去粘接设计的碳化钨车针,清除靠近釉质的复合树脂残留物。
  3. 术后扫描用于实验性体积评估
    1. 重复步骤2.1.1–2.1.5。
       
       
  4. 实验性体积评估
    1. 点击导出,将术前和术后扫描以最高分辨率的表面网格语言(STL)格式导出。
    2. 打开合适的软件,点击重组
    3. 通过点击导入,将术前和术后扫描数据上传至软件中。
    4. 点击排列,采用最佳拟合法对术前和术后扫描进行叠加。
    5. 点击分析,以可视化术前与术后扫描之间的体积变化。在工具部分选择区域,选取可能发生体积变化的牙齿部位。分别使用线性和体积测量软件工具——距离工具和体积分析工具——分析体积变化。
      1. 工具部分点击距离,对牙体组织缺损和复合树脂残留进行线性定量,并以颜色显示(例如:无变化区域:绿色,组织缺损:蓝色和紫色,多余材料:黄色和红色,图3B)。使用左侧的颜色条对线性体积变化进行量化。此外,将光标置于相关牙齿部位,查看左侧框中显示的精确光标距离。
      2. 工具部分点击体积分析,对牙体组织缺损和复合树脂残留进行体积定量。查看左侧框中显示的体积变化数值。

结果

使用荧光诱导透射(FIT)法可使大多数复合树脂材料比正常牙体组织显得更亮(图2图5)。因此,FIT不仅适用于复合树脂材料的检测,还可广泛用于去除复合树脂材料,特别是在正畸托槽脱粘以及外伤后夹板拆除过程中后牙部位的树脂清除1,2,24,25,26,27,28,29,30,

讨论

传统的照明方式(例如牙科综合治疗台的照明灯)在识别复合树脂修复体方面是一种不理想的诊断工具。为了通过传统照明获得更准确的诊断,必须借助放大设备、干燥牙面,甚至费力地清洁牙齿。即使在理想条件下,传统照明似乎仍显不足。一项研究表明,传统照明可能导致复合树脂修复体和正常牙体组织的误判33。相比之下,FIT 方法在多个方面表现出优越性。FIT 是一种具有高准确性、可重复性和重现性的诊断工具1,2。即使存在唾液或生物膜等阻碍因素,也不会影响 FIT 的检测结果1
