Extended Sleeve Resection

Extended sleeve resection is a lung-sparing thoracic surgical procedure that removes a centrally located tumor with involved bronchus and, when necessary, an adjacent pulmonary artery, while preserving functional lung tissue. During surgery, the affected airway is divided beyond the lesion, the diseased segment is excised, and the bronchial ends are joined directly; vascular reconstruction may restore pulmonary artery continuity. This approach can achieve complete tumor removal with less loss of lung tissue than pneumonectomy in selected patients, but it requires careful assessment of resection margins, cardiopulmonary reserve, and anastomotic healing. It is used mainly for centrally located lung cancers and supports individualized, organ-preserving surgical planning.
