Motor Mapping

Motor mapping is the process of identifying brain regions and neural pathways that control voluntary movement, providing a functional view of motor organization in the nervous system. In neuroscience, researchers use methods such as functional magnetic resonance imaging, electroencephalography, or direct cortical stimulation to link neural activity with specific movements; stimulation can transiently activate or disrupt a cortical site, revealing its role. Motor mapping supports studies of motor planning, execution, recovery after brain injury, and neural plasticity. Clinically, it can help guide neurosurgical decisions by locating essential motor areas while preserving function, and it provides a basis for evaluating rehabilitation and brain-computer interface strategies.

Here, we describe a standardized protocol for motor mapping using nTMS combined with diffusion tensor imaging (DTI)-based reconstruction of the corticospinal tract (CST). The protocol is reproducible, clinically feasible, and readily integrable into routine clinical workflows, providing a robust and valuable framework for motor pathway assessment, neuroplasticity research, and rehabilitation planning.
