Microrna Liquid Biopsy

MicroRNA liquid biopsy is a minimally invasive method that analyzes small, noncoding microRNAs circulating in blood or other body fluids to obtain information about cancer. Tumor and surrounding cells release microRNAs within extracellular vesicles, such as exosomes, or in protein complexes, protecting them from degradation and creating disease-associated expression patterns. After biofluid collection, researchers isolate and profile these molecules using techniques such as quantitative PCR or sequencing. In cancer research, microRNA liquid biopsies support biomarker discovery, early detection, tumor classification, prognosis, and treatment monitoring, while offering a potential alternative to repeated tissue sampling and a foundation for more personalized cancer care.

MicroRNA (miRNA) are short, regulatory RNA transcribed from introns—non-coding regions of a gene—or intergenic regions—stretches of DNA present between genes. Several processing steps are required to form biologically active, mature miRNA. The initial transcript, called primary miRNA (pri-mRNA), base-pairs with itself forming a stem-loop structure. The aqueous humor is a high-yield liquid biopsy for retinoblastoma, intraocular cancer that cannot be biopsied in vivo due to the risk of extraocular spread. Herein, a method for safely extracting aqueous humor via clear corneal paracentesis and steps for genomic analysis to identify prognostic biomarkers are presented.

This protocol describes the feasibility to perform miRNA profiling in exosomes, released in plasma of NSCLC patients, through a commercial exosome isolation kit with Proteinase K and RNAse treatments, in order to avoid circulating miRNAs contamination and evaluate their biomarker features in NSCLC.
