Rectal Submucosal Injection

Rectal submucosal injection is an endoscopic technique that delivers a solution beneath the rectal mucosa, where it can create a localized tissue cushion or provide targeted exposure to an experimental agent. A needle passes through the mucosal surface into the submucosal layer, and injected fluid separates the mucosa from the underlying muscularis propria, helping confine the material and lift tissue away from deeper structures. In cancer research, this method supports the removal or treatment of superficial lesions, localized delivery of drugs or biologics, and experimental studies of colorectal tumor development and tissue responses. Its precision can improve procedural safety and help evaluate site-specific therapies.

Source: Joseph Donroe, MD, Internal Medicine and Pediatrics, Yale School of Medicine, New Haven, CT While its usefulness in cancer screening is debated, the male rectal examination remains an important part of the physical exam. The exam is indicated in selected patients with lower urinary tract symptoms, urinary and/or fecal incontinence or retention, back pain, anorectal symptoms, abdominal complaints, trauma patients, unexplained anemia, weight loss, or bone pain. We developed a new ex vivo model that applies constant tension to the porcine gastric specimen. This development made it possible to evaluate the performance (the height and duration of the submucosal elevation) of various SIMs accurately. The detailed setup methodology of this new model is explained.

Here we demonstrate our protocol for isolation of basal and submucosal gland duct cells from mouse tracheas. We also demonstrate the method of injecting stem cells into the dorsal mouse fat pad to create an in vivo model of submucosal gland regeneration.
