Patient Evaluation

Patient evaluation is the systematic assessment of an individual’s health, symptoms, functional status, and clinical needs, forming the foundation for safe, informed care. Clinicians combine the medical history and patient-reported concerns with objective findings from physical examination, vital signs, laboratory studies, and imaging to identify abnormalities, develop a differential diagnosis, and assess risk. Evaluation may occur during routine screening, acute illness, chronic disease management, or follow-up, with findings guiding treatment decisions and referrals. Repeated assessment also reveals changes over time, supports monitoring of therapeutic outcomes, and improves communication among patients and healthcare professionals.

Resting-state functional-connectivity MRI has identified abnormalities in patients with a wide range of neuropsychiatric disorders, including epilepsy due to malformations of cortical development. Transcranial Magnetic Stimulation in combination with EEG can demonstrate that patients with epilepsy have cortical hyperexcitability in regions with abnormal connectivity.

Evaluation of the teaching process enables the nurse to determine if the patient's learning needs were met and if training was effective. If the expected outcomes are not met, the care plan is revised, and additional education or reinforcement is provided. Nurses can ask questions after the session or obtain feedback to assess the patient's understanding of the topic. Nurses can use several methods to evaluate patient outcomes. For example, oral questions can assess cognitive learning, patient...
